About this role

Job summary

We are seeking an experienced and compassionate Admiral Nurse to join Coventry & Rugby GP Alliance. You will deliver specialist, person-centred and relationship-centred dementia care to people living with dementia and their families across Coventry & Rugby, helping to improve the experience and outcomes of those affected by dementia.

Working closely with carers, families and health and social care professionals, you will provide expert assessment, evidence-based interventions, education and support for individuals with complex dementia-related needs. You will also share specialist knowledge and best practice with colleagues and partner organisations, promoting high-quality dementia care and supporting the development of skills and confidence across services.

The role includes supporting the delivery of key quality and performance indicators relating to dementia care, leading on reflective practice, and applying the Admiral Nurse Competency Framework at a specialist level. The successful candidate will have excellent clinical, therapeutic and communication skills, with a passion for delivering high-quality, evidence-based dementia care and improving the experiences of people with dementia and their families.

Main duties of the job

The post holder will be responsible for delivering specialist relationship-centred dementia care to people living with dementia and their carers/families, including undertaking comprehensive bio-psychosocial assessments, developing person-centred care plans, providing evidence-based interventions, emotional support, and advocacy for those with complex needs. The role will involve working collaboratively with health, social care and voluntary sector partners to coordinate care, support care transitions, and improve outcomes for people affected by dementia.

The post holder will also provide specialist advice, consultancy, education and training to colleagues and other professionals to promote best practice in dementia care. They will contribute to the development of dementia services, support the delivery of the organisation's dementia strategy and quality objectives, maintain accurate clinical records, and ensure care is delivered in accordance with relevant legislation, national guidance and organisational policies. The role requires the application of the Admiral Nurse Competency Framework at a specialist level to enhance the quality of dementia care and improve the experience of people with dementia and their families across the organisation.

Job description Job responsibilities

Main Duties and Responsibilities:

Clinical

Advise other professionals on the care of people with dementia and their carers/families with complex care needs, from peri-diagnosis to post bereavement support.Provide specialist nursing assessment, using the Admiral Nurse Assessment Framework for carers/families affected by dementia (within service spec)Support identification and management of co-morbidities and frailty for people with dementia and their carers/families.Develop and/or support, in partnership with other professionals, person-centered care plans for people with dementia including positive risk managementDeliver relationship and family-centered approaches to care, which meet the needs of people with dementia, carers/families and staff.Provide a range of evidence based bio-psychosocial interventions and emotional support to promote mental/physical health and wellbeing for people with dementia and their carers/families.Support people with dementia, carers/families and staff to understand and respond to changes in behaviour and relationships as a result of dementia.Ensure that all legal requirements are considered and adhered to where appropriate to patients and carers, including; Mental Capacity Act 2005, Deprivation of Liberty Safeguards (DoLS) 2009 & Safeguarding Adults 2011Act as an advocate for people with dementia and their carers/families to promote their rights and best interests.Participate in case conferences and best interest meetings when required.Work collaboratively with health, social and voluntary sector services to achieve coordinated care.Manage care transitions and support people with dementia and their carers/families in order to reduce frequency of admissions and length of stay.Ensure that care is delivered in accordance with relevant local and national guidelines and all relevant Coventry & Rugby GP Alliance policies.Recognise the limits of own competency and professional boundaries and make appropriate and timely referrals to other services where required.Ensure that clinical records are up to date and maintained and stored appropriately in line with local and national guidance.

Education & Support

Support innovative dementia training and education to staff within the host organisation at all levels, to improve staff attitudes, knowledge, skills and confidence, in line with best practice and guidance.Participate in and/or support formal training events promoting best practice in dementia care and disseminating Admiral Nursing work.Provide advice on the delivery of evidence based best practice in dementia care.Act as a clinical role model to others, including student nurses, and mentoring other staff as required.Support the development and delivery of work streams as part of the host organisations Dementia Strategy.Work in partnership with colleagues to provide support and information to families, including advance care planning, best interest decisions and end of life care.Work with key local and national stakeholders to develop more integrated care pathways and holistic models of care, through dissemination of specialist skills and knowledge.

Evaluation and Audit

Support the audit and evaluation of quality improvements and initiatives aimed at improving the delivery of dementia care within Coventry & Rugby GP Alliance.Actively participate in the collection of quantitative and qualitative data to support the evaluation and audit of the Admiral Nurse Service.Maintain clinical records using the chosen data recording system and provide verbal and written feedback appropriately and as requiredEnsure recorded information and data recorded is relevant, accurate, and complete and captured in a timely manner.Be fully aware of and comply with the Data Protection Act (1998) and Caldicott principles in relation to protecting the confidentiality of patient and service-user information and enabling appropriate information-sharing.

Professional

Ensure all nursing practice is carried out in accordance with the agreed policies and procedures of Coventry & Rugby GP Alliance, Dementia UK and the NMC Code.Attend and actively participate in practice development, clinical supervision, and continuing professional development supported by Dementia UK and underpinned by the Admiral Nurse competency framework.Develop own competence through use of the Admiral Nurse Competency Framework and portfolio development using resources provided by Dementia UK.To maintain professional registration in line with NMC guidance, including re -validationContribute to the development of policies, procedures and protocols relevant to dementia and Admiral Nursing within Coventry & Rugby GP Alliance.Ensure compliance with Trust clinical governance requirements.Participate in clinical supervision / mentoring within Coventry & Rugby GP Alliance where required.Maintain up-to-date knowledge of evidence-based recommendations on supporting people with dementia and their carers/families.Be aware of current research related to dementia treatments/interventions and support people with dementia/carers/families to get involved in research where appropriate.

Person Specification

Experience Essential

Post-registration clinical experience of working with people with dementia and their carers/supporters in different settings. Experience of recent involvement in practice/ service development. Experience of working with groups and individuals in a variety of roles and settings. Demonstrated experience of working as an autonomous practitioner.

Desirable

Experience of working with patients/ clients to develop practice. Experience of audit/ evaluating clinical practice. Experience of working in the community. Experience of palliative care and long term conditions.

Qualifications Essential

Registered Nurse (RNMH/RGN/RNLD). Formal knowledge of dementia/ older people acquired through clinical post-graduate training/ education.

Desirable

Additional nursing qualification. Diploma / Degree/ level qualification in dementia / older peoples care. Mentorship Qualification. Relevant teaching qualification.
