Job summary
We are looking to recruit an experienced and motivated Practice Nurse to join our friendly and supportive multidisciplinary team. We are looking for someone who is patient-centred, proactive and passionate about high-quality care.
You will work collaboratively within a multidisciplinary team to deliver evidence based care to our practice population and play a key role in health promotion, long term condition management, immunisations, cervical screening, wound care and health promotion. This is an excellent opportunity to work in a progressive practice with a strong focus on quality, safety and patient experience.
Main duties of the job
As a practice nurse you will deliver high-quality patient centred care across a range of clinical areas within general practice. Your day-to-day work will include supporting patients with long term conditions such as asthma, chronic obstructive pulmonary disease (COPD) and diabetes, carrying out routine reviews, and promoting self-management and healthy lifestyles.
You will be responsible for providing core nursing services including immunisations, cervical screening and wound care as well as supporting infection prevention control within the practice. You will work closely with the GPs, healthcare assistants and the wider multidisciplinary team to ensure safe, effective and coordinated care for patients.
The role involves maintaining accurate clinical records, supporting recall and screening programmes and contributing to quality improvement such as Quality and Outcomes Framework (QOF).

Undertake diagnostic health screening and health surveillance
Contribute and promote improved health and prevent disease in individuals and groups
Assess problems presented opportunistically by patients
Manage programmes of care for patients with chronic disease (when appropriate training has been undertaken)
Work in accordance with agreed medical/nursing/practice protocols/patient group directives, with referral to Nurse Manager or GPs as appropriate
Support Practice Development Plans
Record keeping adhering to NMC guidelines
Ensure awareness of statutory and local clinical protection procedures, including systems of referral. Ability to recognise signs and symptoms of abuse or vulnerability and act appropriately.
Apply infection control measures within the Practice according to local and national guidelines
Ability to obtain and document informed consent (either verbal or written)
Confidentiality of information gained at work must be always preserved
To work only in accordance within NMC code of Professional Standards of Practice and Behaviour for Nurses, Midwives and Nursing Associates
Key Duties and Responsibilities
Clinical Care & Patient Management

Health and Safety
The post-holder will implement and lead on a full range of promotion and management their own and others health and safety and infection control as defined in the Practice Health & Safety Policy, the Practice Health & Safety Manual and the Practice Infection Control Policy and Published procedures. This will include (but will not be limited to):
