About this role

Job summary We are looking for an enthusiastic, experienced and dedicated Practice Nurse to join our friendly, forward thinking primary care team. This is an excellent opportunity for a motivated nurse who is passionate about delivering high quality patient care and contributing to the ongoing development of our clinical services. The successful candidate will play a key role in providing a wide range of nursing services within the practice, including routine clinics and chronic disease management.

Main duties of the job

Routine Clinics

Cervical Screening - Perform cervical cytology in line with national guidelines, ensuring high standards of patient care and accurate documentation. Childhood & Adult Immunisations -Deliver vaccinations safely and effectively, following current immunisation schedules and maintaining up to date knowledge of vaccine protocols. Wound Care - Provide assessment, treatment, and ongoing management of acute and chronic wounds, including dressings, suture removal, and patient education.

Chronic Disease Management

Manage and support patients with long term conditions such as asthma, COPD, diabetes, hypertension, and cardiovascular disease. Undertake annual reviews and personalised care planning. Use evidence based practice to deliver high quality care and promote patient self management. Work collaboratively with the wider clinical team to ensure continuity of care and effective person centred management.General Duties

Maintain accurate, timely clinical records in accordance with practice policies. Participate in clinical audits, quality improvement initiatives, and multidisciplinary meetings. Contribute to the development of nursing services and practice protocols. Ensure infection control standards and safeguarding responsibilities are upheld. Provide compassionate, patient centred care at all times.

Person Specification

Qualifications Essential

Registered Nurse - NMC Experience in primary care or a strong interest in developing skills in this area. Competence in cervical screening, immunisations, and wound care or willingness to undertake training Strong communication, organisational, and clinical decision making skills. Commitment to ongoing professional development. Driving licence - for home visits and to be able to work across both our sites- Woolston Lodge Surgery and Chartwell Green Surgery. Desirable Skills

Desirable

Experience in chronic disease management clinics. Knowledge of SystmOne Knowledge of Quality Framework Outcomes (QOF)
