Job summary
We are seeking an enthusiastic, reliable, highly motivated individual to the role of Practice Nurse. You will be joining a well-established team in a well-established practice. The ideal candidate will be an experienced Practice Nurse and ideally have worked in Primary Care before, with an interest and knowledge in Diabetes or Respiratory.
Main duties of the job
Make professionally autonomous decisions and provide safe evidence based patient care Evaluate patient test results in conjunction with the patient and their GP and ensure that the appropriate course of reagent is initiated Initiate the role of diagnosis for patients suspected to have a chronic disease e.g. COPD, asthma, diabetes and CHD , referring to other clinicians as appropriate Manage programmes of care for patients with chronic disease by planning providing and evaluating care under agreed guidelines and patient group directions. Maintain disease registers in liaison with administrative staff. Undertake annual review of patient understanding and ability to self-manage Provide comprehensive travel health for patients prior to travel including malaria prophylaxis, safe sex, sun protection, food hygiene, first aid and emergency medication, health insurance and vaccinations Administer child and adult immunisations and vaccinations in accordance with national and local programmes. Ensuring safe storage, rotation and disposals of vaccines To undertake procedures including venipuncture, glucose tolerance tests, BP management, urinalysis, peak flow, spirometry and cytology. Provide support and chaperoning to the wider team Promote health and wellbeing going general education advice on diet, contraception, smoking, chronic disease management, exercise, etc. Initiate and carry out programmes of health screening as agreed with the practice clinical team

We are a large forward-thinking practice with more than 17,000 patients, based in a purpose built building the centre of Chorley. We have regular practice meetings, with a structured meeting rota including whole staff meetings, clinical meetings, safeguarding meeting and protected time for training. We have a multidisciplinary, clinical team made up of 6 GP Partners, 5 Salaried GPs and 4 Advanced Nurse Practitioners, who are supported by a wider team of Paramedics, Pharmacists, First Contact Physios, a range of specialised Nurses and Health Care Assistants. We are a training practice and have a number of junior doctors working at site, including FY2s, ST2s and ST3s. The practice benefits from a well-structured management team which is led by a Practice Business Manager. Person Specification
Experience Essential
Current NMC registration and eligibility to work in the UK Experience working in line with the NMC Code of Professional Conduct Proven experience in general practice nursing, including: Childhood immunisations and Womens health. Experience of Nurse-led diabetes care including initiating and managing patients on insulin/GLP1s OR respiratory including skilled in performing and interpreting spirometry Experience implementing clinical guidelines and protocols Knowledge of health promotion and preventative care Strong IT skills, including Microsoft Office (Word, Excel, email) Excellent communication, organisational, and record-keeping skills High standard of numeracy and administrative accuracy Ability to work independently, prioritise workload, and use sound clinical judgement Effective team player within a multidisciplinary environment Professional, empathetic, and patient-centred approach Understanding of and commitment to confidentiality and data protection Flexible, reliable, and responsive to change Commitment to continuous professional development, Primary Care, and NHS values Commitment to equality, diversity, and inclusion
