About this role

Job summary

We are seeking an enthusiastic, reliable, highly motivated individual to the role of Diabetic Specialist Nurse. You will be joining a well-established team in a well-established practice. The ideal candidate will be an experienced Practice Nurse with exceptional diabetic knowledge, or a Diabetic Specialist Nurse who has previously worked in secondary care services.

Main duties of the job

Deliver high-quality, evidence-based care to patients with diabetes in a primary care setting Run autonomous nurse-led diabetes clinics, including assessment, diagnosis support, and ongoing management Initiate, review, and optimise diabetes treatment plans, including insulin and other injectable therapies Provide structured education and self-management support to patients and carers Monitor and manage long-term conditions, including complications and comorbidities associated with diabetes Interpret clinical data (e.g. HbA1c, blood glucose readings, lipid profiles) and act accordingly Undertake annual diabetes reviews and contribute to QOF targets and clinical audits Work collaboratively with GPs, practice nurses, HCAs, and the wider multidisciplinary team Liaise with secondary care and specialist services where appropriate Promote health education, prevention, and lifestyle interventions Maintain accurate, contemporaneous clinical records in line with professional and legal standards Support the development and implementation of practice protocols and guidelines for diabetes care Contribute to staff training and act as a resource for diabetes knowledge within the practice Maintain up-to-date professional knowledge and participate in continuing professional development

About us

We are a large forward-thinking practice with more than 17,000 patients, based in a purpose built building the centre of Chorley. We have regular practice meetings, with a structured meeting rota including whole staff meetings, clinical meetings, safeguarding meeting and protected time for training. We have a multidisciplinary, clinical team made up of 6 GP Partners, 5 Salaried GPs and 4 Advanced Nurse Practitioners, who are supported by a wider team of Paramedics, Pharmacists, First Contact Physios, a range of specialised Nurses and Health Care Assistants. We are a training practice and have a number of junior doctors working at site, including FY2s, ST2s and ST3s. The practice benefits from a well-structured management team which is led by a Practice Business Manager. Person Specification

Experience Essential

Current NMC registration and eligibility to work in the UK Registered Nurse with significant experience in diabetes care (primary or secondary care) Recognised diabetes qualification or equivalent experience (e.g. diploma/module in diabetes) Independent Nurse Prescriber qualification (V300) Experience running nurse-led diabetes clinics and managing complex cases Competence in initiating and adjusting diabetes treatments, including insulin and injectable therapies Strong knowledge of current diabetes guidelines and best practice Experience in patient education and supporting self-management Ability to interpret clinical data (e.g. HbA1c, glucose monitoring, lipids) and act accordingly Excellent communication and interpersonal skills Strong organisational skills with the ability to work independently and prioritise workload Effective team player within a multidisciplinary setting High standard of clinical record-keeping and IT skills (clinical systems and Microsoft Office) Commitment to confidentiality, safeguarding, and professional standards Flexible, reliable, and responsive to service needs Commitment to continuous professional development and NHS values

Desirable

Experience in managing complex or poorly controlled diabetes, including Type 1 diabetes Previous experience working as a Diabetic Specialist Nurse in primary care Experience contributing to service development, audits, and quality improvement initiatives Teaching or mentoring experience, including supporting junior staff or students Knowledge of QOF, NICE guidelines, and local diabetes pathways Experience using Emis
