Job summary
We are seeking an enthusiastic, committed, and ideally experienced Practice Nurse to join our friendly, supportive and motivated practice in Putney, South West London.

Main duties of the job
Along with our GPs and other clinical staff, the post holder will be responsible for ensuring the delivery of safe and effective nursing care to the whole practice population, clinically delivering evidence-based practice for patients with acute and chronic conditions and management and preventative nursing interventions to all patients. You will work collaboratively with the whole general practice team to meet the needs of patients, supporting the delivery of procedures.

Heathbridge Practice hosts a great team of GPs, Nurses, Healthcare Assistants and Administrative staff working together to better the lives of the residents of Putney, London. We are highly regarded in Wandsworth for delivering high quality patient care and patient centred care.
Wandsworth borough hosts a large young population, and so we have fewer frail or elderly patients than other parts of the country. This also means our chronic disease registers are lower than average.
Our GPs operate a triage model that combines telephone and online triage. Our nursing team offers a large number of face to face appointments that can be either requested by the patient or booked directly by the GP on the basis of the triage conversation.
Our nursing team currently comprises of 3 Practice Nurses, 3 Healthcare Assistants, all of whom are led by our Advanced Nurse Practitioner Partner. To complement the practice team, we have 15 GPs, 2 Practice Pharmacists, 2 Physiotherapists, a Social Prescriber, a Mental Health Link Worker, a Physician's Assistant, and a great team of dedicated administrative personnel. The practice is a training facility for both GP registrars & nursing students.

Qualifications Essential
RGN/RN Qualified NMC registration

Previous General Practice experience Mentor or teaching qualification Post graduate diploma or degree (Chronic Disease Management) Nurse Prescriber Family Planning qualification
