Main duties of the job
As a All Age Continuing Care Nurse, you will work autonomously as a Nurse Assessor / Case Officer, managing a defined caseload. You will be responsible for coordinating holistic assessments, contributing to eligibility decision-making, and supporting the planning, review and monitoring of care packages and complex care pathways in partnership with providers and multi-disciplinary colleagues.
You will ensure that care is person-centred, outcome-focused and represents best value, while maintaining quality, equity and consistency across the system. The role requires strong clinical judgement, effective communication skills and the confidence to work across organisational boundaries.

Undertake comprehensive health and needs assessments in line with the National Framework. Provide case management and oversight. Contribute to eligibility recommendations and provide professional clinical advice. Coordinate and review complex care packages, ensuring quality and value for money. Work closely with colleagues across health and social care. Support individuals and families, providing clear information and managing sensitive discussions. Maintain accurate, timely and legally compliant records. Contribute to service development, quality improvement and audit activity.
Person Specification
Qualifications Essential
Professional qualification and current registration with the NMC or SWE Qualified to post graduate level or equivalent level demonstrable experience. Valid UK driving licence.
Experience Essential
Extensive experience of working with adults and children in a social care or health care setting. Significant experience of working in partnership with service users, carers, service providers and other professionals. Significant experience of developing Self Directed Support Plans to meet individual need. Significant experience and expertise of assessing, planning and implementing care packages and liaising with providers. Experience of interpreting national and local policies related to All Age Continuing Care and case management. Experience of reviewing costs associated with care packages and making recommendations. Experience of line management, appraisal, mentorship and coaching, etc... working withing relative organisational policies. Experience of ensuring compliance with documentation and accuracy of collation of information. Experience of participating in audit and service improvement. Experience of working autonomously within national policy and organisational policies.
