Job summary
Elmwood Family Doctors is a large, forward-thinking General Practice serving approximately 15,500 patients across two sites in Holmfirth and Meltham, on the edge of the beautiful Pennine countryside.
We are seeking an enthusiastic and motivated Salaried GP to join our supportive clinical team on a fixed-term contract of up to six months, working up to six sessions per week.
You will be joining a well-established multidisciplinary team of 10 GP Partners, 2 Salaried GPs, 4 Advanced Nurse Practitioners, Practice Nurses, Healthcare Assistants, and an experienced management and administrative team. Main duties of the job
Our Practice operates Monday to Friday, with flexible working patterns designed to ensure service requirements are met while supporting individual preferences wherever possible. To meet our service needs, the successful candidate will be required to provide cover on Tuesdays and Fridays, with a third day negotiable. Working hours will be agreed with the successful candidate and will include participation in some extended access sessions.
We use SystmOne as our principal clinical system.

Key Clinical Responsibilities
The successful candidate will practice in accordance with GMC standards, accepted good medical practice, and the clinical policies and procedures of Elmwood Family Doctors.
Key responsibilities will include:
Providing high-quality, patient-centred care through face-to-face and telephone. Working to an agreed timetable, including routine and urgent appointments, home visits where required, and the management of clinical correspondence, results and prescription requests. Assessing, diagnosing and managing patients presenting with both acute and long-term health conditions. Making professional and autonomous clinical decisions, including the management of undifferentiated and complex presentations. Undertaking health promotion and disease prevention activities, including identifying risk factors and supporting early intervention. Developing and reviewing personalised care plans in partnership with patients and in line with current clinical guidance and practice protocols. Maintaining accurate, contemporaneous and high-quality clinical records. Contributing to the achievement of Quality and Outcomes Framework (QOF) indicators and other quality improvement initiatives. Prescribing safely, effectively and in accordance with local and national prescribing guidance. Participating in clinical audit, quality improvement activities and service development projects. Supporting the delivery of enhanced services and contributing to clinical governance, education and the continual review of practice policies and protocols. Working collaboratively with colleagues across primary, secondary and community care to ensure patients receive appropriate and timely support and referrals. Contributing positively to the multidisciplinary team and participating in appraisal, revalidation and continuing professional development activities. The post holder will be expected to undertake the full range of duties associated with a GP working in primary care and to comply with the requirements of the General Medical Council, Royal College of General Practitioners, NHS England and other relevant professional and regulatory bodies.

Support the Practice complaints procedure, participating fully in the investigation and resolution of complaints where required and promoting a culture of learning and continuous improvement. Contribute to maintaining a safe, effective and responsive service by identifying and escalating issues relating to quality, patient safety and clinical risk. Demonstrate a commitment to equality, diversity and inclusion, treating patients, carers and colleagues with dignity, respect and compassion at all times. Work flexibly and collaboratively to support the ongoing needs and development of the Practice and the wider patient population.
