About this role

Job summary

We are looking for an organised, proactive and highly capable Neighbourhood Team Coordinator to provide dedicated coordination, project, governance and administrative support to the PCN management team.

The role will provide additional capacity to the PCN management team, supporting the PCN Clinical Director, Strategy & Transformation Manager and Neighbourhood Lead with the coordination and delivery of their work.

The postholder will take responsibility for a significant proportion of the day-to-day coordination and administration associated with PCN, neighbourhood and transformation activity. This will enable the PCN management team to focus on clinical leadership, strategic development, neighbourhood relationships, transformation and delivery.

This is not simply an administrative role. We are looking for someone who can coordinate projects and activities, bring the right people together, maintain momentum, follow up actions, support governance and quality processes, and build effective working relationships with a wide range of stakeholders.

The successful candidate will be naturally organised and proactive, able to understand what needs to happen next and ensure that agreed actions are progressed.

Main duties of the job

Key responsibilities

1. Leadership, administrative and coordination support

Provide a central coordination and administrative function for the PCN

2. Meeting and stakeholder coordination

3. Neighbourhood Integrated Care support

Support the PCN management team as the PCN develops its approach to Neighbourhood Integrated Care.

4. Project and transformation coordination

Support the delivery of PCN projects, service developments and transformation programmes.

5. Women's health projects

The post holder will support the development and delivery of women's health projects and initiatives across the PCN, supporting the Womens Health Lead

6. Digital Transformation Support

7. Governance, CQC and quality

Provide coordination and administrative support to maintain effective PCN governance, quality and compliance arrangements.

We are an established Primary Care Network (PCN) in South East Telford, serving approximately 38,000 patients across our three member practices: Stirchley Medical Practice, Court Street Medical Practice and Woodside Medical Practice.

Established effective systems for tracking meetings, actions, projects, risks and stakeholder contacts.

Ensured meetings are well organised and attended by the appropriate stakeholders.

Developed a reliable system for following up contacts and actions arising from neighbourhood discussions.

Supported effective communication between the PCN and wider neighbourhood partners.

Supported the coordination of multidisciplinary and partnership activity.

Strengthened the PCN's governance and documentation processes.

Established an organised approach to CQC evidence and action tracking.

Helped maintain momentum across key PCN transformation and neighbourhood projects.

Supported the PCN management team as new approaches to Neighbourhood Integrated Care develop.

Provided timely and accurate information to support the management team's work.

Become a trusted, reliable and proactive source of coordination and support for the PCN management team.

Person specification Experience working in an administrative, coordination, project or programme support role. Experience organising meetings involving multiple stakeholders. Experience maintaining action logs, project plans, registers or similar systems. Experience following up actions and ensuring deadlines are met. Experience working with a range of internal and external stakeholders. Experience managing competing priorities. Experience handling confidential and sensitive information. Experience producing accurate written records and documentation. Experience working collaboratively as part of a team. Experience using initiative to organise and progress work within agreed responsibilities Desirable experience Experience working within primary care, the NHS, health or social care. Experience supporting CQC, governance, compliance or quality processes. Understanding of Primary Care Networks. Understanding of the developing role of Neighbourhood Integrated Care. Awareness of health inequalities, patient engagement and digital inclusion initiatives Experience supporting transformation or service improvement projects. Experience working across organisational boundaries. Experience supporting governance or partnership meetings. Knowledge of NHS structures and terminology. Good knowledge of Emis and the reporting functions.
