We are looking for an enthusiastic, friendly and organised Care Co-ordinator to join our Care Navigation Team.
As a key member of our team, you will be one of the first points of contact for our patients. You will help patients access the right service or clinician, provide a welcoming and professional service, and ensure that our administrative processes run smoothly.
This is an excellent opportunity for someone who enjoys working with people, is able to remain calm and professional in a busy environment, and takes pride in providing excellent patient service.

Main duties of the job Main duties of the role
The successful candidate will be responsible for:
Providing a friendly, efficient and effective care navigation service to patients and visitors.
Dealing with general enquiries and assisting patients with appointments and other Practice services.
Managing patients who require urgent attention in an appropriate and efficient manner.
Supporting new patients with Practice arrangements, registration procedures and requirements.
Processing home visit requests in accordance with Practice policy, accurately recording all relevant information and referring requests to the appropriate clinician.
Receiving and making telephone calls, ensuring messages are recorded accurately and passed to the appropriate member of the team promptly.
Updating patient information and electronic records accurately and in accordance with data protection legislation.
Processing prescriptions in line with Practice procedures.
Maintaining accurate and up-to-date electronic patient records.
Advising patients of charges for private services, processing payments, issuing receipts and completing relevant documentation.
Assisting with opening and closing the Practice when required, including activating/deactivating the alarm and ensuring the premises are secure.

The role involves a combination of patient-facing, administrative and telephone duties, including managing appointments and enquiries, processing home visit requests and prescriptions, updating patient records and supporting patients with Practice procedures and services.
You will need to communicate confidently and sensitively with patients, including those who may be distressed or require urgent assistance. Accuracy, confidentiality and a commitment to following Practice policies and procedures are essential.
We are looking for someone who is friendly, organised, flexible and reliable, with excellent communication and interpersonal skills. You should be comfortable working both independently and as part of a multidisciplinary team, be able to prioritise your workload and remain calm under pressure.
Previous experience working with the public and in an administrative role is essential. Experience within a healthcare or general practice setting, together with knowledge of SystmOne, would be advantageous.

Qualifications Essential
Qualifications - GCSE English and Maths at Grade C/4 or above. Experience - Experience of working with the general public. - Experience of carrying out administrative duties. Skills - Excellent written and verbal communication skills. - Clear, polite and courteous telephone manner. - Strong general IT skills. - Effective time management and organisational skills. - Ability to work effectively both as part of a team and independently. - Good interpersonal skills. - Good problem-solving and analytical skills. - Ability to follow policies and procedures. Personal Qualities - Polite, confident and approachable. - Flexible and co-operative. - Motivated and enthusiastic. - Able to use initiative and judgement, including knowing when to seek advice or assistance. - Forward-thinking and proactive. - High levels of integrity and loyalty. - Sensitive and empathetic when dealing with patients in distressing situations. - Ability to work effectively under pressure. - Positive attitude and willingness to contribute to the Practice team.
