Main duties of the job
Lead proactive, evidence-based nursing care for people living with long-term conditions, supporting self-management and preventing avoidable deterioration.Provide high-quality treatment room and preventative care services across the age range, guided by current best practice and professional standards.Offer clear health education and personalised advice, helping patients understand their conditions and treatment plans.Work closely with GPs, HCAs, pharmacists and wider MDT colleagues to coordinate safe, joined-up care.
Use clinical systems to record accurately, monitor outcomes and contribute to quality improvement, safety audits and practice targets.Play an active role in safeguarding, infection prevention and maintaining a positive, patient-centred culture at the surgery.

Job description Job responsibilities Clinical Duties
Deliver care for patients with chronic diseases, including asthma, COPD, diabetes, and hypertension
Provide cervical cytology (smears) in accordance with national screening guidelines
Administer vaccinations and immunisations (childhood, adult, and travel)
Perform wound care, dressings, and removal of sutures/staples
Assist with family planning, contraceptive advice, and NHS health checks
Undertake ECGs, blood pressure checks, and other routine procedures
Support infection prevention and control within the practice
Provide lifestyle advice and health promotion to improve patient outcomes
Collaborative Working
Work closely with GPs, HCAs, pharmacists, and other clinical staff to deliver coordinated care
Participate in multidisciplinary meetings and case reviews as needed
Refer patients appropriately within the team and to external services
Governance and Safety
Ensure accurate, timely documentation in clinical records (using SystmOne)
Follow safeguarding protocols and contribute to safeguarding vulnerable patients
Adhere to infection control policies, cold chain management, and health and safety procedures
Engage with clinical audits, QOF targets, and quality improvement activities
Maintain NMC registration and meet revalidation requirements
Person Specification
Qualifications Essential
Registered General Nurse (RGN) with current NMC registration Completion of a Practice Nurse Induction Programme or General Practice Nurse (GPN) Foundation Course Experience in a general practice or primary care setting Competent in chronic disease management, cervical cytology, immunisations, and wound care Excellent communication, record-keeping, and time management skills
Desirable
Experience using SystmOne Additional training in travel health, smoking cessation, contraception, or spirometry Awareness of QOF and its relevance to nursing care
