Job summary
Wootton Vale Healthy Living Centre arelooking for an enthusiastic and skilled practice nurse to join our friendly and forwardthinking primary care team. This position is offered on a full pr part-time basis with flexible working options available.

As a key member of our clinical team, you will deliver a wide range of nursing services and support patients in managing chronic health conditions. We are particularly interested in someone who can take a proactive, holistic approach to patient care.
Main duties of the job
Management and review of patients with long term conditions (e.g., diabetes, CVD, hypertension asthma and COPD) Supporting long term condition clinics and QOF-related work Providing high-quality general practice nursing services such as immunisations, wound care, cervical screening, health checks and lifestyle advice Working autonomously while contributing to the wider multidisciplinary team Supporting practice initiatives to improve patient outcomes and access Supporting and mentoring colleagues including HCAs and Nurse Associates
About us
We are a friendly surgery with a list size of over 18,000 patients, over two sites, Wootton Vale, (MK43 9JJ), and branch site Shortstown Surgery, (MK42 0GS). We have good appointment access, which translates to happy patients, and a strong teamwork ethos, that includes an established clinical pharmacy team, GPs, nurses, physician associates, clinical liaison and admin teams. We are also a tier-2 sponsoring practice and would support any applicants requiring a tier 2 visa.

Person Specification
Qualifications Essential
A relevant nursing degree. Current NMC registration with an active PIN.
Desirable
Independent Nurse Prescriber (V300) qualification
Experience Essential
An experienced practice nurse working in a GP or primary care setting Skilled in longterm condition management with confidence running LTC clinics preferably diabetes and cardiovascular. Knowledgeable in relevant clinical guidelines and best practice A great communicator with a compassionate, patient-centred approach Organised, proactive and able to work both independently and within a team
Desirable
Competence in managing and prescribing for LTCs Experience using GP clinical systems such as EMIS SystmOne (if applicable)
