About this role

Job summary

This post is a fixed term/secondment for 6 months from start date to meet the needs of the service. If you are interested in applying for the secondment position, you must obtain permission from your current line manager prior to applying for this post.

In this role, you'll provide person-centred therapeutic support to older people experiencing mental health difficulties across community settings, care homes, and memory assessment services.

You'll carry out specialist assessments, reviews, and interventions for people with complex needs, including dementia, cognitive impairment, and other mental health conditions, making sure care is effective, evidence-based, and as least restrictive as possible.

You'll work flexibly across services--supporting patients, carers, and staff while also getting involved in service development and quality improvement.

You'll also help plan and oversee care for patients in both primary and secondary care, including those living in the community and in care homes.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Main duties of the job

The main duties of this role involve working closely with older people and their families to provide compassionate, person-centred support, including having sensitive conversations around diagnosis and care needs in a reassuring and respectful way.

You'll carry out specialist mental health assessments, including memory and functional assessments, and manage a varied caseload across community and care home settings. You'll deliver practical, evidence-based care for people with dementia and other mental health conditions, including care planning and regular review.

You'll support timely diagnosis, manage risk, and respond appropriately to crises. Working as part of a wider team, you'll collaborate with GPs, OTs, psychologists and social care colleagues to ensure care is coordinated and effective.

Alongside maintaining your professional standards and ongoing development, you'll support junior staff and contribute to a positive team culture. You'll also be involved in service improvement, sharing knowledge, and helping to develop high-quality dementia care.

The role includes working within key legal frameworks, ensuring safe practice, safeguarding, accurate record-keeping, and maintaining confidentiality at all times.
