About this role

Job summary

Applications are invited for the post of Specialty Doctor for the Community Care of the Elderly team in Ebbw Vale.

In this role you will be working as part of a larger multidisciplinary team to deliver specialist care to our older and frail patients with a focus on enabling care closer to home. You will be joining the team at an exciting time where you will be given ample opportunities to help shape the future of community COTE services. You will be working with Frailty colleagues to streamline and support early discharge from hospital into the community.

Your principal place of work will be Ysbyty Aneurin Bevan but you may be required to work across other sites within the Health Board according to service need.

The organisation is currently developing an Older Persons pathway to improve patient outcomes and experiences from home to hospital. The implementation of this strategic piece of work is likely to impact doctors working within the COTE Directorate in the future and there is an expectation that job plans will reflect the new pathway. The job plan within this job description has elements of this change, however, candidates should be aware that further changes are likely to take place in order to successfully implement the pathway fully across the Health Board.

Main duties of the job

The main responsibilities of the post are the effective and appropriate medical care of patients in the community including acute assessments and participation in community-based rehabilitation.

Responsibility for inpatient admission, day-to-day management, co-ordination of inpatient management under the supervision of the Consultant/Specialist Grade and/or in close liaison with community teams. Key role in discharge planning and co-ordinating approaches with relatives and other agencies. Liaison with patients and relatives/carers and members of the multi-disciplinary team to ensure the patients receive a high-quality service and are all kept informed of progress as appropriate. Responsibility for contemporaneous record keeping, identifying progress and plans in patients' case notes. Participation in Consultant rounds and facilitating multidisciplinary meetings. The completion of accurate and timely discharge summaries, progress reports ensuring General Practitioners are informed of any impending discharges, changes of medication and death. To assist Consultants in the development of inpatient, outpatient and community-based services. The completion of e-discharge summaries on a timely basis via the Clinical Workstation system. To actively participate in multi-disciplinary audit, clinical effectiveness/health gain initiatives and clinical governance, taking a lead in local clinical audit activities and developing research projects, as appropriate.
