About this role

Job summary

This is an exciting opportunity for an experienced community clinician to join the LMWS management team, with a specific focus on the PCMH element. The role will involve providing managerial link support to the LYPFT clinicians working in LMWS. In addition, other elements of the role will involve providing clinical advice and supporting wider service improvements from an operational perspective. As well as being an integral part of supporting with the wider transformation of community mental health services in Leeds.

Main duties of the job

The Clinical Team Manager (CTM) will provide clinical leadership and professional oversight within the service. The role will support staff through formal and informal clinical supervision, professional guidance, and clinical decision-making, ensuring safe and effective practice. The CTM will also contribute to risk management, safeguarding, incident reviews, and the implementation of service improvements to enhance patient outcomes. The CTM will work alongside other LMWS managers and support in a timely and supportive way to ensure the daily smooth operations of the PCMH element.

CTM will also work alongside other CTM's within the wider LYPFT community services as part of the transformation model - transforming community mental health services across the city of Leeds. Working collaboratively with LMWS managers and wider community services, the CTM will support the delivery, embedding, and evaluation of new models of care, helping to ensure high-quality, responsive, and integrated community mental health services across Leeds.

The Leeds and York Partnership NHS Foundation Trust (LYPFT) is the main provider of Mental Health and Learning Disability services in Leeds. We also provide specialist inpatient services in York and some highly specialised services across the country. Person Specification

Qualifications Essential

Relevant core professional qualification and registrations (eg. RMN, RN, SW, OT (Mental Health) Evidence of ongoing and recent post qualification continuous development

Knowledge Essential

A sound understanding of community and primary care mental health environments. Evidence of understanding service improvement and has been involved with local improvement project.

Experience Essential

Understanding and experience of providing clinical leadership and operational management. Evidence of partnership working
