Job summary Are you a passionate Physiotherapist looking to develop your skills across a broad range of community and rehabilitation services? We are seeking a motivated Band 5 Physiotherapist to join our Rapid Response and Intermediate Care Team in Swale. Undertake six-month rotations across Rapid Response, Intermediate Care and the Inpatient Rehabilitation Unit Assess, plan, implement and evaluate physiotherapy treatment programmes for patients with a variety of complex physical health conditions Complete comprehensive, holistic assessments and develop person-centred rehabilitation goals Deliver evidence-based interventions designed to maximise independence and functional ability Manage your own clinical caseload with support from senior colleagues as required Work collaboratively with patients, carers, families and multidisciplinary colleagues to ensure coordinated care Support discharge planning and facilitate smooth transitions between services Maintain accurate and timely clinical records in line with professional and organisational standards Participate in clinical supervision, service development initiatives, audit and quality improvement activities Delegate appropriate tasks to Rehabilitation and Therapy Support Workers Attend team meetings, clinical forums and in-service training sessions Contribute to the continuous improvement of patient outcomes and service deliveryThe Ideal CandidateWe are looking for someone who is enthusiastic about rehabilitation and community-based care and is committed to delivering excellent patient experiences.

You will have:

Essential Degree or Diploma in Physiotherapy HCPC registration as a Physiotherapist (or be awaiting registration as a final-year student) Strong assessment, treatment and clinical reasoning skills Excellent communication and interpersonal abilities Effective organisational and prioritisation skills Good IT skills, including Microsoft Office applications Ability to work independently and as part of a multidisciplinary team Full UK driving licence and access to a vehicle for work purposesDesirable Membership of the Chartered Society of Physiotherapy (CSP) Previous experience in community, rehabilitation, neurology, musculoskeletal or orthopaedic settings Understanding of clinical governance, audit and quality improvement Evidence of continuing professional development

Qualifications Essential

Degree or Diploma in Physiotherapy, HCPC registration as a Physiotherapist, strong assessment, treatment and clinical reasoning skills, excellent communication and interpersonal abilities, effective organisational and prioritisation skills, good IT skills, ability to work independently and as part of a multidisciplinary team, full UK driving licence and access to a vehicle for work purposes.
