Job summary St Gemmas Hospice Community Services is seeking an experienced, compassionate and registered nurse to provide operational leadership to our Community Specialist Palliative Care Team. This is an exciting opportunity to join a highly regarded community service and play a key role in supporting the delivery of safe, responsive, equitable and person-centred specialist palliative and end of life care for patients, families and carers across the community.
Main duties of the job As Operational Team Leader, you will provide day-to-day operational leadership and oversight for the Community Specialist Palliative Care Team. You will support effective service coordination, referral flow, triage arrangements, safe staffing, caseload management, prioritisation, escalation of risk and service responsiveness.
You will line manage and support Band 7 Senior Clinical Nurse Specialists and work through them to support consistent leadership, practice and performance across locality/cluster teams. You will also play an important role in staff wellbeing, professional development, performance conversations, workforce planning and creating a positive, inclusive and psychologically safe team culture.Working closely with the Leeds Hospices Integrated Community Specialist Palliative Care Services Lead, the Nurse Consultant for the City-Wide Inclusion Service, medical colleagues and wider multidisciplinary partners, you will support the implementation of agreed service priorities, pathways, quality improvements and consistent ways of working across the service.

We offer care to patients, families and carers in the Hospices In-Patient Unit, through the St Gemmas community team, and at our Out-Patients.

Experience Essential
Significant and recent post-registration experience in specialist palliative care or equivalent Proven experience and consolidation at Band 6 or above, including senior clinical decision-making and leadership. Proven experience of community-based service delivery. Significant experience of leading and developing clinical teams, including performance management, supervision, appraisal and staff development Proven experience of managing competing operational priorities, including caseload, capacity, staffing or urgent clinical pressures. Proven experience of multidisciplinary and multi-agency working. Proven experience of leading or implementing quality improvement, service redesign, audit or governance initiatives Experience of supporting teams through service change, role development or new ways of working
Qualifications Essential
Registered Nurse professional qualification (Adult). Post-registration qualification or substantial experience relating to specialist palliative care and end of life care. Evidence of leadership or management development, or equivalent experience. Evidence of supporting learning and development in practice or equivalent qualification Advanced communication skills training or willingness to undertake. Evidence of Continued Professional Development. Non-medical prescribing qualification. Hold a full UK driving licence with no restrictions and have access to a reliable vehicle for work-related travel, with the ability to travel independently across the locality as required.
Additional Criteria Essential
Knowledge and understanding of Palliative and End of Life care local and national policies, guidance and priorities and application in clinical practice. Demonstrates in depth knowledge regarding recognition and management of complex symptoms associated with progressive incurable illness Knowledge of evidence / research based practice within specialist palliative care Knowledge of clinical governance, safeguarding, risk management and quality assurance. Knowledge of ensuring equality and inclusion in service delivery within relevant specialty. Experience in implementing quality standards in a clinical healthcare environment. Able to apply specialist palliative care knowledge to support complex symptom management, clinical prioritisation and escalation. Able to provide visible, compassionate and credible operational leadership. Able to manage day-to-day service pressures calmly and effectively and make sound operational decisions. Excellent communication, negotiation and influencing skills. Able to support staff through complex, distressing and emotionally demanding situations. Able to analyse information, identify risks and escalate appropriately. Able to lead and empower others, supporting innovation, service development and change. Able to manage difficult conversations, conflict and competing expectations with compassion, clarity and professionalism Confident in using a range of IT systems (SystmOne) software applications, and technology to support day-to-day work activities. Proficient in Microsoft Office applications, including Outlook, Word, Excel and Teams, with excellent written communication skills. Able to use service data to inform operational decisions, identify risks and support service improvement Able to work independently, use own initiative and work effectively as part of a team. Ability to create psychologically safe, inclusive and supportive team environments Understanding of Neighbourhood Health models
