About this role
Job summary Working with the wellbeing service team, you'll support people to make positive lifestyle changes so that they can improve their health and wellbeing. Based within a person-centred service, you will help individuals identify achievable goals and build the confidence to make lasting changes. This role combines health improvement, behaviour change and physical activity support. You will work with a wide range of people, including those with long-term health conditions, delivering personalised advice, specialist exercise programmes and ongoing motivation. You'll also work closely with healthcare professionals, community partners and support services to provide a joined-up approach to improving health outcomes. What you'll do:
Assess clients' needs and develop personalised action plans to support healthier lifestyles Deliver physical activity sessions, lifestyle interventions and health improvement advice Support and motivate clients to achieve and maintain their health goals Manage a caseload, monitor progress and maintain accurate records Work with healthcare professionals and community organisations to provide coordinated support
Main duties of the job What we're looking for:
