Are you a passionate and motivated Practice Nurse with an interest in diabetes care and long-term condition management?
You will play a key role in delivering high-quality, evidence-based care across chronic disease management, health promotion, screening, and preventative services.
You will be supported by an experienced multidisciplinary team and encouraged to develop your skills, particularly in diabetes care, while contributing to a service that is responsive, patient-focused, and continually improving.

Main duties of the job
Providing clinical care in the treatment room, including wound care, injections, and immunisations.
Supporting the management of long-term conditions such as diabetes, asthma, and hypertension.
Performing routine health checks, screenings, and vaccinations.
Working collaboratively with the Lead Practice Nurse, GPs, and the wider multidisciplinary team.
Contributing to the development and improvement of primary care services in response to increasing demand.
We are looking for individuals with proven Primary Care nursing skills and experience, who can demonstrate safe, evidence-based clinical practice, excellent communication, and a commitment to patient-centred care.
Conduct routine health checks, screening, immunisations, and travel vaccinations, as well as treatment room procedures including wound care and minor procedures. Support patient self-management through education, advice, and health promotion.
Work collaboratively with GPs, the Lead Practice Nurse, and the wider multidisciplinary team to ensure safe, effective, and patient-centred care.
Contribute to service development and quality improvement initiatives, helping to shape and improve the patient experience at the practice.

Deliver nursing care to patients across a range of clinical areas Support the management of patients with long-term conditions Promote health and wellbeing through preventative care and education Work both independently and as part of the wider practice team Job Responsibilities
Clinical Care
Provide high-quality, evidence-based nursing care in line with NICE guidelinesDeliver routine nursing services including:Wound care and dressings, including suture and staple removalCervical cytologyImmunisations (routine, childhood, and travel)ECGs and spirometry Undertake new patient health checks and routine screening Chronic Disease Management
Support the management of patients with long-term conditions, including:
Diabetes Asthma and COPD HypertensionCarry out reviews, monitoring, and patient educationSupport patients to self-manage their conditionsWe welcome applications from Nurses across all specialist fields. Health Promotion & Prevention
Deliver opportunistic health promotion and lifestyle advice Support national and local health campaigns (e.g. vaccinations, NHS health checks) Promote patient wellbeing and disease prevention Infection Prevention & Control
Work in accordance with infection prevention and control policies Support IPC audits and compliance activities as required Medicines Management
Administer medications in accordance with PGDs and scope of practice Provide advice to patients on medication use, side effects, and compliance Patient Management & Administration
Request and process pathology tests as appropriate Maintain accurate, clear, and contemporaneous clinical records Refer patients to GPs or other healthcare professionals when appropriate Team Working
Work collaboratively with GPs, nursing colleagues, and the wider MDT Support colleagues and contribute to a positive team environment Participate in practice meetings and training
Person Specification
Qualifications Essential
Registered Nurse (NMC) Experience in chronic disease management Immunisation and vaccination training Experience of working in primary care Experience managing long-term conditions, particularly diabetes Experience working autonomously within scope of practice Chronic disease management (including diabetes) Wound care and dressings Immunisations and vaccinations Cervical cytology Spirometry and ECGs Pathology testing and results handling Knowledge of evidence-based practice and NICE guidelines
Desirable
Cervical cytology training Experience with audit or quality improvement Experience with elderly or housebound patients
