About this role

Job summary

The Programme Director of Continuing Health Care and Funded Nursing Care is responsible for leading the strategic transformation agenda across the Care Group for Primary Care and Community to deliver high-quality, integrated services that meet the needs of patients and the community.

The post holder will have delegated senior lead reporting to the Executive Director of Nursing for Continuing NHS Healthcare (CHC) across the Health Board, ensuring:

Robust governance and oversight

Full compliance with the National Framework for CHC

High-quality, person-centred care

Effective financial stewardship and value-based commissioning

Clear and consistent reporting and assurance to the Board

The role will act as the senior operational and strategic lead for CHC on behalf of the Executive Director of Nursing, leading system-wide improvement in partnership with Local Authorities and Regional Partnership Boards (RPBs), ensuring alignment with Welsh Government expectations for urgent improvement and national consistency.

The post holder will ensure the Health Board can provide clear assurance on CHC governance, quality, performance and financial risk, supported by robust data, audit and continuous improvement processes.

Main duties of the job

Visionary leadership for the Care Group transformation agenda, ensuring alignment with national healthcare policies, including the CHC National Framework and National Commissioning Framework

Lead the development and delivery of a Health Board wide CHC strategy and improvement plan aligned with National priorities and the emerging national CHC improvement programme

Ensure all CHC governance risk and performance information is synthesised into clear assurance reporting to the Executive Director of Nursing, including escalation of risks, compliance issues and financial pressures

Work collaboratively with other Directors in the development and delivery of a robust Development Plan, identifying current and future service and transformational needs to deliver integrated care. This will require extensive and ongoing research into the approach taken in other areas

Lead on the design, development, and delivery of complex transformation programmes, including clinical service redesign, digital transformation, workforce optimisation and CHC pathway redesign

Responsible for providing strategic overview, leadership and transformational change to enable the effective integration of services. Lead and facilitate the resolution of strategic issues and interdependencies and provide flexible targeted delivery support to the organisation to deliver the Transformation & Improvement priorities.

Person Specification

Qualifications and Knowledge Essential

Master's degree in a relevant field (e.g. Health Management, Business Administration, or equivalent experience). Project / Programme Management qualification (e.g. PRINCE2, MSP) or equivalent experience. Evidence of continuous professional development in leadership, change management, or transformation. Current Healthcare Professional Registrant (NMC / HCPC). Experience of leading Continuing Healthcare (CHC) services, commissioning, or complex care systems. In-depth understanding of the National Framework for CHC and commissioning in Wales. In-depth understanding of the NHS and current healthcare policy, including the challenges and opportunities facing the sector.

Experience Essential

Demonstrable experience of providing board-level assurance on quality, safety, performance and finance. Proven track record of leading large-scale transformation programmes within a healthcare or public sector environment, showing specialist knowledge of transformation and its application to improving healthcare services. Significant experience of working at a senior leadership level within a complex healthcare organisation, including experience of working with executive teams and boards. Evidence of being able to influence and negotiate on complex subject matter decision making. Experience of leading service redesign and improvement initiatives that have delivered measurable improvements in patient outcomes and efficiency, including the writing of relevant complex business plans, policies, and strategies. Demonstrated success in managing change within a multidisciplinary environment, including managing cultural and organisational change using change management methodologies. Experience of working collaboratively with external partners and stakeholders, including commissioners, local authorities, and other healthcare providers. Proven leadership experience at the highest levels within a healthcare or public sector organisation, including strategic decision-making and influencing board-level stakeholders.

Skills and Atrributes Essential

Ability to lead system-wide governance and assurance frameworks. Strong understanding of data, performance management and quality assurance in complex systems. Highly developed leadership skills, with the ability to inspire coach and motivate teams to deliver high quality work. Excellent communication and interpersonal skills, with the ability to build relationships, influence stakeholders, and present complex information clearly and persuasively. The ability to communicate contentious and challenging subjects effectively to multiple professional groups. Strong analytical and problem solving skills, with the ability to make informed decisions in a fast paced environment. Able to rapidly understand subject matter from across different areas e.g. planned and unplanned care and provide an informed opinion using a transformation mindset. Ability to manage multiple priorities and work under pressure to meet deadlines
