About this role

Job summary This role helps young people with ongoing health needs move confidently from children's to adult nursing services, making a real difference to their long-term wellbeing. [810-P1064...tion Nurse | Word] You'll work across children's and adult community nursing teams to make sure transition is planned, joined-up and centred on each young person. The role focuses on coordination, advocacy and quality, helping young people and their families feel informed, supported and listened to at a time of change. What you'll do:

Coordinate smooth transitions for young people from around age 14 into adult services Act as a key contact, tracking progress so no one is lost during transition Chair transition multidisciplinary meetings and follow up agreed actions Support young people and carers to understand choices, consent and capacity Share skills and learning with colleagues to build confidence across teams

Main duties of the job What we're looking for:

A registered nurse with current Nursing and Midwifery Council registration Experience supporting young people or young adults with complex health needs Confidence working across teams and services, using good judgement Strong communication skills, with a calm and inclusive approach An interest in learning, teaching others and improving how services work We welcome applications from people with different backgrounds and experiences and value ideas that help us deliver fair, respectful and inclusive care for all. Person Specification

Qualifications Essential

First Level Registered Nurse with Current NMC Registration Level 5 Teaching/Mentoring/Assessor qualification or equivalent experience

Desirable

Leadership/Management/Coaching qualification

Knowledge Essential

Sound knowledge of transition challenges faced by young people locally Competent and up to date evidence-based knowledge, in theory and practice relating to clinical care of young people/young adults with complex health needs (for example care of enteral feeding devices, CVADs, catheterisation, symptom management and palliative care) Knowledge of national legislation/transition processes and best practice guidance in transition of young people with complex health needs Knowledge of local systems, networks and partnerships to ensure we remain 'system-connected' to enable young people to receive a high quality and well managed experience of transition Knowledge of when and how to escalate concerns about a young person's wellbeing, safety, clinical condition or transition progress Knowledge of safeguarding in practice

Desirable

Sound knowledge of the Mental Capacity Act, consent and best interest decision processes

Experience Essential

Significant experience of working with young people and/or young adults with complex health needs in a community setting Experience of developing teaching packages/training others and assessing clinical competence Experience of advocating for patients with complex health needs Experience of challenging practice to ensure the patient receives high quality care that is personalised. Experience of caseload management including the use of SystmOne to plan, implement and evaluate care Collaborative leadership skills including delegation and performance monitoring, with the ability to influence others positively Evidence of CPD in specific area of work

Desirable

Experience of performing clinical audits/research Experience of policy development (e.g SOPs, Procedures, clinical pathways)
