The successful candidate will deliver a broad range of nursing services including chronic disease management, cervical screening, immunisations and vaccinations, wound care, health promotion and preventative care. You will work closely with GPs, Advanced Clinical Practitioners, Clinical Pharmacists, Healthcare Assistants and the wider Primary Care Network to provide safe, effective and compassionate care.

Main duties of the job
The successful candidate will provide high-quality nursing care to patients within a busy and supportive general practice environment. Working as part of a multidisciplinary team, you will play a key role in promoting health, preventing disease and supporting patients to manage long-term conditions.
Your responsibilities will include delivering routine and preventative nursing services, undertaking clinical assessments, providing evidence-based advice and education, and contributing to the ongoing development of safe, effective patient care. You will work collaboratively with GPs, Advanced Clinical Practitioners, Clinical Pharmacists, Healthcare Assistants and other healthcare professionals to ensure patients receive coordinated, person-centred care.

Essential Requirement
Applicants must have previous experience working as a Practice Nurse within a UK GP Practice. Applications from nurses without General Practice experience will not be considered.

The successful candidate will:
Deliver safe, effective and evidence-based nursing care. Manage and deliver chronic disease clinics in line with current guidelines and QOF requirements. Undertake cervical cytology screening. Administer childhood, adult and travel immunisations and vaccinations. Provide wound care, dressings and removal of sutures. Perform venepuncture and routine health assessments. Deliver health promotion and lifestyle advice. Support minor surgery and specialist clinics where required. Make appropriate referrals to primary, community and secondary care services. Maintain accurate and contemporaneous patient records. Participate in clinical audit, infection prevention and quality improvement activities. Contribute towards achieving practice, PCN and contractual objectives. Work collaboratively with the wider multidisciplinary team to ensure excellent patient outcomes.Professional Responsibilities
Maintain current NMC registration and work within the NMC Code of Conduct. Participate in clinical supervision, training and continuing professional development.
