Job summary

ABOUT THE ROLE

As Care Coordinator Team Lead, you will provide operational and professional leadership to Primary Care Care Coordinators, ensuring patients receive timely, coordinated support across health and care services. Working closely with PCNs and GP practices, you will oversee the systems, processes, and workflows that underpin effective care coordination, including referral management, patient allocation, reporting, and service delivery.

This role focuses on line management, governance, workforce development, and continuous service improvement rather than carrying a routine caseload. You will work collaboratively with multidisciplinary teams and partner organisations to promote joined-up care, improve patient outcomes, reduce health inequalities, and support people to remain independent within their communities.

Main duties of the job

KEY RESPONSIBILITIES

Provide effective line management, professional leadership, and supervision to Care Coordinators.

Ensure staff are supported to deliver high-quality personalised care through structured supervision, appraisal, and development planning in line with national expectations

Provide oversight to ensure Care Coordinators effectively support individuals to navigate and access services across the health and care system.

Ensure care coordination is delivered safely, effectively, and in line with organisational and governance standards.

KEY REQUIREMENTS

Evidence of leadership development relevant to service or workforce management

Training or awareness in personalised care / care coordination approaches

At least3 Diploma in Health & Social Care

Experience of clinical supervision

KEY RESPONSIBILITIES

Leadership and Line Management

Provide effective line management, professional leadership, and supervision to Care Coordinators.

Ensure staff are supported to deliver high-quality personalised care through structured supervision, appraisal, and development planning in line with national expectations.

Develop and maintain a competent and capable workforce aligned to the NHS England Care Coordinator framework, including training, induction, and ongoing CPD.

Create a culture of reflective practice, continuous learning, and professional curiosity.

Manage workforce matters including performance, absence, wellbeing, and recruitment.

Ensure clarity of role boundaries, responsibilities, and expectations within the Care Coordinator function.

Oversight of Care Coordination Delivery

Provide oversight to ensure Care Coordinators effectively support individuals to navigate and access services across the health and care system.

Ensure delivery of personalised, holistic care coordination that focuses on what matters to the individual.

Ensure Care Coordinators assess, monitor, and proactively respond to changing needs, particularly for those with complex conditions or frailty.

Oversee caseload management across the team, ensuring safe allocation, appropriate risk stratification, and equitable workload distribution.

Act as an escalation point for complex, high-risk, or deteriorating patients.

Ensure care coordination activity is proactive, preventative, and supports independence and wellbeing.

Multidisciplinary Team (MDT) Working and System Integration

Ensure Care Coordinators are fully embedded within MDTs and wider multi-agency teams.

Enable and support staff to bring together professionals across services to deliver coordinated, joined-up care.

Strengthen relationships with primary care, community services, secondary care, and voluntary sector partners.

Ensure effective systems are in place to support timely information sharing, communication, and collaboration across organisations.

Promote a coordinated approach that reduces duplication and fragmentation of care.

Personalised Care and Patient Empowerment

Ensure consistent application of personalised care principles, including shared decision-making and supported self-management.

Enable Care Coordinators to build effective relationships with individuals and act as a key point of contact.

Ensure individuals feel supported to navigate services and are empowered to take an active role in their health and care.

Clinical Governance, Quality and Risk

Ensure care coordination is delivered safely, effectively, and in line with organisational and governance standards.

Monitor and improve the quality of assessments, care planning, documentation, and communication.

Ensure safeguarding processes are embedded, understood, and consistently applied.

Lead on incidents, complaints, and learning to drive improvement.

Support audit, data collection, and service evaluation to improve quality, consistency, and outcomes.

Operational and Service Management

Provide oversight of the operational systems and processes underpinning care coordination.

Monitor demand, capacity, and service performance, taking action to address pressures or variation.

Ensure effective referral management, triage, prioritisation, and allocation processes are in place.

Ensure workflows support timely, coordinated responses to patient need.

Optimise the use of workforce and resources to meet population health needs.

Contribute to service planning and delivery of organisational and system priorities.

Workforce Development and Supervision

Ensure robust and consistent supervision arrangements are in place to support safe and effective practice.

Support Care Coordinators to meet competency expectations and progress within their role.

Ensure training and development align with national standards for personalised care and care coordination.

Reducing Health Inequalities and Improving Outcomes

Ensure the service proactively supports populations at greatest risk, including those with long-term conditions and frailty.

Use population health approaches to identify, prioritise, and manage need.

Support proactive and preventative care models that reduce hospital admissions and crisis interventions.

Promote equitable access to coordinated care and reduce variation in outcomes.

Communication and Stakeholder Engagement

Maintain effective communication across teams, services, and partner organisations.

Act as a key escalation and leadership contact for operational and service issues.

Ensure clarity and consistency in communication of service expectations, processes, and updates.

Service Improvement and Innovation

Support Care coordinators in the tools required for continuous improvement of care coordination pathways and delivery models.

Use data, feedback, and audit to identify opportunities for improvement and drive change.

Support the implementation of national frameworks and best practice to improve consistency and outcomes.
