Job Summary
Pathfields Medical Group is looking for an experienced Practice Nurse to join our friendly and supportive multidisciplinary team.
This role would particularly suit a nurse with experience or a strong interest in Learning Disability health reviews and chronic disease management, who is passionate about improving outcomes for patients with complex needs.
You will play a key role in delivering high-quality primary care services, supporting patients with long-term conditions and promoting preventative healthcare within our community.
You will be supported by an experienced Lead Nurse who provides clinical leadership, mentorship, and guidance to the nursing team, helping to ensure high standards of patient care and ongoing professional development.
Working Pattern
29.5 hours per week
Monday: 08:00 18:00 (30 min lunch) Tuesday: 08:00 18:00 (30 min lunch) Wednesday: 08:00 13:00 Thursday: Non-working day Friday / Saturday: Alternate days 08:00 13:30 Hours and days may be flexible, we welcome a discussion

Qualifications Essential
The Ideal Candidate Will Have: Registered Nurse with current NMC registration Experience working in primary care as a Practice Nurse Experience managing chronic disease and long-term condition reviews Experience delivering immunisations and vaccination programmes Experience or interest in Learning Disability health reviews and assessments Understanding of clinical governance, evidence-based practice and PGDs Strong communication and teamwork skills Ability to work independently while supporting the wider multidisciplinary team
