About this role
The Case Manager for Veteran Services provides essential case management services and support to veterans and their families at risk of or currently experiencing homelessness. Coordinating closely with the Sioux Falls VA Homeless Case Managers and VA Liaison, the Case Manager helps veterans evaluate all available resources and ensures a comprehensive approach to care. This role involves developing individualized action plans for each participant, holding regular meetings to assess progress, and gathering detailed information about the educational, financial, social, and emotional histories of the veterans and their families. Participation in advisory meetings with veterans and professional staff is required, as well as individual client meetings to address skills and progress.
