About this role
Medical Counselors play a vital, hands-on role supporting individuals with intellectual and developmental disabilities and autism by coordinating and accompanying them to medical appointments. You will build trusting relationships while ensuring clear communication between individuals, healthcare providers, and the program nurse. Responsibilities include tracking health needs, reinforcing care plans, supporting follow-up treatments, and promoting overall wellness. In collaboration with the interdisciplinary team, you help individuals understand their care, advocate for their needs, and maintain continuity of services.
