We are hiring a Licensed Practical Nurse for a Home Health position providing skilled nursing care to patients in their homes in the Mechanicsburg, PA area. This position is eligible for a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year!
What You’ll Do:
• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient assessments including vital signs, medication management, wound care, and overall health status.
• Educate and support patients and caregivers on disease management, medication administration, and prescribed treatments to encourage adherence and confidence in care routines
• Maintain accurate, timely, and compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

LPN Qualifications
• Active Licensed Practical Nurse (LPN) license in the state you work
• Two years of LPN experience and the clinical expertise to deliver quality patient care
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
