About this role

Job summary

We are seeking an experienced Diabetes Specialist Nurse to join our Inner London Diabetes Service on a maternity cover basis.

The service covers the London tri-boroughs of Hammersmith & Fulham, Kensington &Chelseaand Westminster, and works within a multidisciplinary teamcomprisingconsultants, specialist nurses,dietitiansand administrative staff.

We work in partnership with general practitioners and other healthcare professionals to provide tailored support and guidance to people living with diabetes. Ourmain officebase is centrallylocatedat Lisson Grove, and we currently deliver clinics across nine sites within the boroughs.

Main duties of the job

Act as a key member of the multidisciplinary Community Diabetes Service, planning and delivering high-quality integrated diabetes care within the community. Provide specialist assessment and treatment for patients with diabetes, liaising effectively with other agencies and healthcare professionals and maintainingaccurateand contemporaneous clinical records. Lead the planning, implementation and ongoing support of specialist nursing interventions, using project management principles to ensure effective delivery. Organise and deliver diabetes education programmes for healthcare professionals. Utilise project management skills and work collaboratively with service users, healthcare providers and professionals to plan, develop, implementand evaluate service developments and diabetes interventions. This work should be informed by current evidence-based guidelines and policies, local population needs, Trust strategicobjectives, governmentinitiativesand national priorities. Contribute actively to the development and implementation of nursing clinical standards, policiesand procedures within the multidisciplinary Community Diabetes Service. Undertake clinical audits and/or research projects as directed by the Service Lead and present findings and reports to relevant groups within the Trust. Maintain and enhance professional knowledge and expertisethrough continuous professional development activities.

Person Specification

Education/Qualification Essential

Degree in Nursing or equivalent. Current registration with the NMC, Showing a registered General Nurse Relevant post registration training in Diabetes eg: Masters or diploma in diabetes management. Independent Nurse Prescriber (or willing to undertake)

Desirable

Mentorship training. Supervisory skills training Mentorship training. ? Supervisory skills training

Experience Essential

Substantial experience in managing complex diabetes cases. Experience in managing a clinical case load. Experience in the training of students. Experience in organising and implementing training/teaching. programmes for healthcare professionals and clients Excellent group education and presentation skills. Experience of conducting audits and/or research. Excellent group education and presentation skills. ? Experience of conducting audits and/or research. Experience in supervising others. Experience working in a multi-disciplinary team.

Desirable

Experience working in Community settings. Experience with working with change behaviour techniques.

Skills & Knowledge Essential

Ability to undertake leadership role in clinical speciality. Advanced skills in the assessment and treatment of clients with diabetes Ability to work as a team member and promote collaborative multidisciplinary and multi-agency working. Advanced knowledge of evidence-based practice associated with diabetes. Ability to interpret health and social policies and guidelines and to translate into practice. Ability to problem solve, prioritise and demonstrate clinical reasoning based on experience and knowledge Ability to interpret health and social policies and guidelines and to translate into practice. ?
