Job summary
We are seeking an enthusiastic and experienced DiabetesSpecialist Practitioner to join our friendly and forward thinking primary care team. This is an exciting opportunity to lead and develop our diabetes service, providing high quality, patient centred care to people living with diabetes and associated long term conditions.
The successful candidate will work autonomously within their scope of practice, managing all aspects of diabetes care including diagnosis, annual reviews, medication optimisation, insulin initiation and titration, patient education, and the prevention and management of complications. They will work closely with GPs, pharmacists, nurses, healthcare assistants, and the wider multidisciplinary team to deliver evidence based care and improve patient outcomes.
The role will also involve supporting achievement of national quality standards, reducing health inequalities, identifying patients requiring proactive intervention, and contributing to service development and quality improvement initiatives. Experience in managing associated conditions such as hypertension, chronic kidney disease, cardiovascular risk, and obesity would be advantageous.

Main duties of the job
The Diabetes Specialist Practitioner will be responsible for delivering high quality, evidence based diabetes care within the GP practice, supporting patients to achieve optimal health outcomes and effectively manage their condition.
Main duties include:
Providing comprehensive care for patients with pre-diabetes, Type 1, Type 2 and other forms of diabetes. Undertaking annual diabetes reviews, including assessment, care planning and ongoing monitoring. Initiating, adjusting and optimising diabetes treatments, including insulin and injectable therapies, within the scope of practice and prescribing authority. Monitoring and managing diabetes-related risk factors and identifying patients at risk of complications. Providing education and lifestyle advice to support patients with self-management and improve long term outcomes. Interpreting clinical investigations, including blood glucose monitoring, HbA1c results and other relevant tests, to guide treatment decisions. Working closely with GPs and the wider multidisciplinary team to deliver coordinated, patient centred diabetes care. Referring patients to specialist services where appropriate and ensuring effective follow up. Maintaining accurate clinical records and supporting the achievement of diabetes-related quality standards and targets. Participating in clinical audit, service development and quality improvement activities to enhance the practices diabetes service.

Working collaboratively with GPs and the wider multidisciplinary team, the successful candidate will promote the early identification and management of diabetes, helping to prevent complications and improve long term outcomes. The role includes interpreting clinical investigations, monitoring treatment effectiveness, making appropriate referrals, maintaining accurate clinical records, and supporting the practice in achieving national diabetes quality standards and targets. The post holder will also contribute to service development and quality improvement initiatives to enhance diabetes care across the practice.
Person Specification
Qualifications Essential
Qualifications (Essential) * Registered healthcare professional (NMC, HCPC or GPhC as appropriate). * Evidence of specialist training in diabetes management. * Independent Prescribing qualification or willingness to work towards. * Evidence of continuing professional development relevant to diabetes care. * Competent in managing long term conditions within primary care.
