About this role

Job summary

To provide administrative support for the Mental Health Practitioner Service, Health & Wellbeing Coach Service and the Social Prescribing Service.

To ensure that all patients referred in are logged and triaged to the appropriate member of each service.

Main duties of the job

To contact patients and provide a service introduction to ensure that expectations are clear.

To book initial consultations on behalf of service staff and send reminders to patients.

Other agreed administrative tasks as requested and agreed with the service leads, such as meeting notes.

To support the Service Leads and H&WB Manager with service reporting, to provide visibility to the GP practices around work done.

Sutton PCNs

Sutton Primary Care Networks is the collaborative vehicle for the four Sutton PCNs and the former Sutton GP Federation working together as a single entity. We are a Community Interest Company that aims to reinvest surpluses back into general practice and primary care for the benefit of our practices and the people who form the communities we serve.

Sutton has four PCNs whose boundaries are based on historical relationships and groupings of organisations. There are some similarities of residents health and care needs unique to a particular PCN, and many more needs that are shared across all four PCNs.

Our four PCN represent all 21 GP surgeries in Sutton. Their collective footprints cover all of the London Borough of Sutton and its residents who are able to access and benefit from the services and improvements offered by Primary Care Networks.

Primary Care Networks (PCNs) are a key part of the NHS Long Term Plan, with all general practices being required to be in a network by June 2019, and CCGs/Integrated Care Boards (ICBs) being required to commit recurrent funding to develop and maintain them.

The networks will have expanded neighbourhood teams which will comprise a range of staff such as GPs, pharmacists, district nurses, community geriatricians, dementia workers and Allied Health Professionals such as physiotherapists and podiatrists/chiropodists, joined by social care and the voluntary sector.

Responsibilities

1. Referral triage for each service.

2. Booking in and carrying out introduction calls for referrals and sending back inappropriate referrals to referrers.

3. Booking in the initial consultation for each patient with the appropriate service team member.

4. Day to day management and oversight of appointment books, active patient load as well as waiting lists.

5. Sending appointment reminder texts.

6. Oversight of DNAs and focused work on improving the current rates for each service.

7. Manage and oversight of complaints & learning events to ensure a reduction of issues especially around patient bookings.

8. Last minute staff sickness appointment cancellations and communication.

9. Ongoing admin support in the office for team members who travel between many different sites/practices such as letter printing and posting letters to patients.

10. If capacity allows

a. Supporting service audit completion

b. Uploading compliments to teamnet

c. Managing the H&WB Teamnet page

d. Supporting at all-staff meetings.

e. Helping to organize/attend community events/sessions.

11. Any other reasonable duties required by the Service Leads or Health & Wellbeing Manager in support of the services.

This team member would be based in head office at Thomas Wall.

Team working

Work with the Health & Wellbeing Service Leads and team members. Work with the practice & Sutton PCNs teams to ensure delivery of services and programmes. Actively engage in the performance cycle to identify training and support requirements.

Person Specification

Experience Essential

Excellent written and oral communication skills Good organisational skills Approachable Experience learning new systems. Experience working with a wide range of people from different backgrounds. Experience of working in multi disciplinary teams

Desirable

Experience working in health, community or voluntary sector

Qualifications Essential

A good knowledge and use of Microsoft Excel and file based systems. Knowledge of Equal Opportunities, Diversity & Inclusion policies.

Desirable

A good knowledge of EMIS.
