About this role

Job summary

This post of PCN Lead Nurse is for an experienced and qualified GP Practice Nurse who has senior or advanced practice qualifications including clinical and leadership skills. Provide leadership and management support to the Nursing and HCA teams within the PCNs, including participation in the annual appraisal and performance management processes. Liaise with the Sutton PCNs Executive to recruit, develop and manage the Nursing and HCA team.

Collaborate with the Clinical Governance Quality and Safety (CGQS) Committee to manage patient expectation, service quality and organisational safety in line with CQC requirements.

Main duties of the job

Lead and oversee Infection Prevention and Control in Patient Services, managing outsourcing provision as appropriate (please see section on Infection Prevention & Control). Ensure that the Nursing and HCA team working collaboratively with other members of the PCN team across the four PCNs to enable effective synergy. Work in accordance with the Nursing and Midwifery Code of Conduct (NMC). Work 1 evening 4-8pm or Saturday mornings as required Chair meetings as required, including but not limited to nurse team meetings, medication safety and IPC reviews.

Job description Job responsibilities

Provide professional advice, work in collaboration with PCNs, Practice Managers and GP Practices, in the development and delivery of integrated services

Responsible for the development and delivery of a consistently high quality service, promoting the team as a positive place for staff experience and development.

Participate in clinical audits and research projects, and implements changes as required, including the development, and updating of protocols, guidelines and procedures in conjunction with the Head of Quality

Assist Sutton PCN initiatives and projects by delivering professional expertise within the contemporary primary care landscape in order to support effective at scale working.

Evaluate and analyse data to interpret clinical findings using evidence based knowledge, to support clinical decisions.

Deliver enhanced clinical care in the context of continual change, challenging environments, different models of care delivery, innovation and rapidly evolving technologies to manage complex interventions.

Work within national and local protocols, where these exist, while recognising the scope of their practice.

Delegate appropriately to members of the Team

Demonstrates accountability for team performance and service delivery ensuring alignment with the organisations objectives

Provides supervision to nurse associates, general practice nurses, health care assistants and trainees as required

Supervise clinical assessments and consent agreements and consult on the provision of enhanced practice advice on vaccines and immunisations across a primary care network. Address g vaccine hesitancy and possible contra-indications to vaccination in partnership with a wide range of stakeholders. Supervise the review of patients medical history and address any potential adverse reaction risks; provide patients with information on actions to be taken in the event of adverse reactions. Supervise vaccination delivery (oversee paperwork and vaccine confirmation activity, ensure compliance with current national IPC guidelines ). Assess and sign off competence of new vaccinators. Contribute to the development and implementation of operational plans and strategies designed to achieve local and national targets and objectives. Ensure team goals, work plans and timelines for achievement are adhered to, evaluate own and team achievement against targets and establish mitigation following discussion with the Senior Team Participate in the teaching and mentoring of other members of the nursing team and PCN staff as appropriate. Assist in developing, implementing and updating clinical and non- clinical policies as related to PCN clinical service delivery. Investigate complaints and significant events, assisting in providing timely, comprehensive responses and contribute to multidisciplinary learning events. Provide clinical input to the PCN operations team in the onboarding of new services as systems develop. Contribute and oversee the implementation of the training strategy for the nursing and HCA team. Develop systems to safeguard the efficiency and effectiveness of the nursing team including the development of standard operating procedures (SOP) and clinical protocols, where required.

Person Specification

Qualifications Essential

Current NMC Registration RGN. Registered with the Nursing & Midwifery Council. A post graduate qualification at level 7 or above relevant to their area of enhanced practice, public health, and population health management. Working at an enhanced level of practice as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework. A non medical prescribing qualification would be beneficial to this role.

Desirable

Chronic Disease Management Qualifications (e.g. Diploma, Degree) ENB 998 Teaching and assessing in clinical practice (or equivalent e.g. mentorship)

Experience Essential

Minimum of 5 years post graduate experience in a community or primary care setting Experience of working across the PCN, in services that enhance the core general practice offer to deliver the key responsibilities outlined. Experience of working with a multi disciplinary team Experience of implementing protocols and clinical guidelines Experience of managing complex and changing conditions in patients.
