Job summary
Mid Yorkshire Teaching Trust, Wakefield Council and Kirklees Active Leisure are working collaboratively with Yorkshire Cancer Research and Sheffield Hallam University to implement the Active Together model to individuals diagnosed with cancer in Wakefield and North Kirklees.
The Active Together programme will run alongside the existing diagnostic and treatment pathways to ensure patients are optimally prepared for treatment and beyond. It aims to maximise resilience to treatment, improve longer term health, minimise side effects and empower patients to be partners in their healthcare. The programme combines physical activity, nutritional support and psychological support alongside the clinical pathway.
The programme has a staged implementation into all relevant cancer pathways. Patients will receive individual assessments and be triaged into a support pathway suitable to their needs. This may be delivered in Mid Yorkshire Hospitals, selected council facilities closest to the patients home or may be a self-management pathway.
With support from senior colleagues, the post-holder will represent psychology at the Active Together MDT and will deliver the psychology component of the programme.
Main duties of the job
Deliver Interventions: Provide targeted, evidence-based psychological support as part of a holistic prehabilitation programme.
Support Adjustment: Help patients manage anxiety, uncertainty, and emotional distress associated with their diagnosis and upcoming treatments.
Enhance Engagement: Use psychological insights to improve patient engagement, motivation, and adherence to physical activity, nutrition, and wellbeing components of prehab.
Group Work: Develop and deliver workshops or group interventions focused on coping strategies, resilience, and emotional well-being.
Collaborative Practice: Work closely with the wider MDT to ensure psychological considerations are embedded throughout each patients prehabilitation journey.
Monitoring & Evaluation: Maintain accurate clinical records, contribute to outcome monitoring, and support ongoing development of the programmes psychological offer.
Training & Consultation: Provide psychological expertise, consultation, and training to colleagues to ensure psychologically-informed care.
