Job summary

An exciting opportunity has arisen within the Childrens Community Nursing Team for a Band 6 Children's Community Nurse - Permanent - Full Time.

The Childrens Community Nursing Team are a friendly, welcoming Team of Nurses who are passionate about meeting the health needs of children & young adults referred into the Service.

The Childrens Community Nursing Team offer specialist nursing support to families who have children or young people with nursing or medical needs in their homes or other community setting such as schools and nurseries. The Team provides support to specialist tertiary teams in providing nursing cares away from a hospital setting.

The role will include supporting with:

Nutrition and Enteral feedingOncology/chemotherapy treatment and supportRheumatology care and supportRespiratory supportWound careParental/carer trainingSymptom managementDiagnosis and case managementLiaison (local & regional)Equipment provision/home delivery equipmentFacilitate seamless care from tertiary & secondary care to primary care.

The role is Community based; you will need to hold a full driving licence plus have access to a car.

The working hours at present are:

Monday Friday 09:00am 17:00pm, On Call over a weekend/Bank Holidays.

Going forward the service is to revert to a 7 day service.

Main duties of the job

The post holder is required to provide effective patient centred care by performing individual assessment of care needs, developing, implementing and evaluating programmes of care that meet the needs of the patient and their families/carers referred into the Service.

The role is multidimensional and requires the post holder to integrate and fulfil all the identified dimensions. As a registered nurse all activities provide opportunities for acting as a role model for staff.

To be responsible for caseload management & clinical leadership in the care of children and young people with palliative care and specialist nursing needs.

Utilise their advance clinical expertise within a multi-disciplinary or multi-agency context, advising on clinical care and ethical issues to other professionals

To be responsible for the line management of junior and unqualified staff.

Community experience BSc (Hons) Community Specialist Practitioner (Community Children's Nursing) or willingness to undertake Qualification in Teaching & Assessing or willingness to undertake

Competencies:

Tracheostomy Portacaths, CVL, Broviac Lines – Line cares plus obtaining bloods Ventilation Nasogastric Tubes, Gastrostomy, Jejunostomy Wound Cares IM & SC Injections End of Life Care Seizures Medicines Management Intravenous Medication Access to a vehicle with the ability to travel independently in line with requirements and a valid UK Driving Licence.
