Job summary
Sloan Medical Practice are offering and exciting opportunity for an enthusiastic practice nurse to join our practice team. The position has become available following the practice deciding to expand the nursing team. We are offering a permanent, part-time position for up to 25 hours.
Sloan Medical Centre is a well-established, four partner PMS (Personal Medical Services) training practice that provides high-quality, patient-focused care supported by a wider team.
We are an 14,000 plus patient practice, high QOF achievers, supportive and collaborative working environment, excellent nursing, management, and administrative support, active in the teaching and supervision of Medical Students and Registrars.
We use System One clinical system and accurx total triage
Salary: Dependant on experience
Hours: 25 each week
Pension: NHS
Holidays: 6 weeks plus bank holidays (pro-rata)
Study leave: 1 week
Main duties of the job
The role needs to be worked flexibly in line with the practice opening hours
Will work collaboratively within the general practice team to meet the needs of patients, support the delivery of the Quality & Outcomes Framework (QoF) and any other performance monitoring systems.
About us
Our nursing team includes 2 Advanced Clinical Practitioners, 3 Practice Nurses, a Student Nurse, 2 HCAs and a Phlebotomist. The expansion of the nursing team will enable them to take responsibility of our womans health service we currently offer our patients.
Job description Job responsibilities
Assist in and perform routine tasks relating to patient care. Undertake and promote cervical cytology. Provide a comprehensive family planning service and sexual health service. Undertake Spirometry and have undertaken relevant training. Manage newly diagnosed patients with Asthma, COPD & Diabetes.
Undertake Chronic disease management and prevention Asthma, COPD, Diabetes, Hypertension, Hypothyroidism, IHD, Stroke, LD.
Perform wound care using aseptic technique and general wound care.
Undertake opportunistic screening of BP checks, height, weight, BMI, urinalysis, glucose monitoring and smoking status.
Administer injections under an individual prescription or patient group direction
Offer a holistic approach to travel health including comprehensive advice for patients prior to travel.
Competent in anaphylaxis and resuscitation techniques.
Child Health and Child Protection, including immunisations.
Promote health and well-being, through the use of health promotion, health education, screening and therapeutic communication skills.
Refer and use social prescribing teams.
To undertake home visits.
To clearly communicate the health status of the patient using appropriate terminology, technology and format.
INR monitoring if agreed by a shared care protocol.

Nursing & Midwifery NMC Registered Qualified to undertake Cytology
Experience Essential
Experience of working as a Practice Nurse
Desirable
Previously worked in a General Practice setting undertaking long terms conditions i.e Diabetes, COPD, Asthma etc
