Job summary

Croydon Health Services are recruiting a Team Leader for our Enhanced Care team.

The Enhanced Care team has been in operation for almost 9 years and has evolved during that time to include all acute adult inpatient area's and the Emergency Department. The Team Leader position provides clinical and managerial leadership to a team of 12 Registered Mental Health or Learning Disability Nurses and 16 Health Care Assistants. The team receive referrals for Enhanced Therapeutic Observations and Care (ETOC) intervention, complete assessments to ascertain if ETOC is required to support the patient maintaining their safety and develop care plans which support least restrictive, person centred care offers.

The Enhanced Care Team Leader is a important member of the Croydon Health Services team and we're looking for someone who can work collaboratively with all teams who make referrals including speciality professionals. We're also looking for someone who can respond proactively to concerns from referrers and Clinical Specialists and deliver patient centred, holistic care.

Working as a Mental Health Nurse within an acute trust environment requires the ability to practice effectively across professional boundaries and support mental health care delivery by a predominantly non-mental health workforce.

Main duties of the job

1. To undertake clinical and management responsibility for nurses and health care workers within the Enhanced Care team.

2. Have an overview on the day-to-day running of the Enhanced Care team, ensuring a safe and effective service based on the assessed need and resource availability..

3. To provide expert advice and leadership to all members of the Enhanced Care team on the provision of care for this patient group.

4. To liaise proactively with ward leaders and matrons on the provision of Enhanced Care to provide and promote seamless patient-centred care.

5. To ensure appropriate referral, discussions and decisions with ward leaders to optimise patient care and improve patient outcomes and experience.

6. Ensure patients are appropriately assessed, have collaborative care plans that are implemented and evaluated and are appropriately discharged from the service.

7. Work in collaboration with the Lead Nurse for Dementia, Mental Health Leads and Substance Use leads on the development and delivery of therapeutic care planning for patients receiving Enhanced Therapeutic Observations and Care (ETOC)

Person Specification

All Essential

Registered Nurse (Mental Health) Experience of working with other disciplines in caring for people with mental illness Leadership/ management qualification or experience Ability to manage staff according to Trust or organisation policy Knowledge of quality, standard setting, benchmarking and audit. Experience of managing staff roster, sickness and absence Use of MS Office packages including word, excel and power point

Desirable

Mentorship/ Practice assessor Experience of working across different settings within speciality of mental illness

Registration Essential

Active NMC registration
