About this role

Job summary

We have an exciting opportunity to recruit a band 6 Stroke Nurse Practitioner for a period of 8 month to work with/alongside our existing team.

This is a senior Nursing/Midwifery role within the organisation and there is an expectation that the post holder will fully participate in the leadership and delivery of high quality patient care within the Care Group.

The post holder will be a practitioner within Stroke speciality.The prime function of the role is to provide clinical expertise in supporting the delivery of high standards of nursing care for patients within the Stroke pathway.

Work collaboratively with the multi professional team to develop and maintain a quality service for patients within speciality

To provide expert clinical nursing, education and advice in all aspects of the speciality.

Facilitate an improvement in the quality of health and outcome of care to this client group.

To act as a source of expertise and knowledge for patients, carers and health professionals as well as other agencies involved in this speciality.

To work with multi-disciplinary teams both in hospital and the community, in the provision of optimal care, to improve quality of life.

The post-holder is responsible for supporting the continuous improvement of quality of services and safeguarding high standards of care by sustaining an environment in which excellence in clinical care can flourish.

Main duties of the job

Promote clinical excellence through his/her practice in the Stroke speciality and the wider organisation.

Lead clinically and professionally making autonomous decisions as required utilizing analytical skills to respond appropriately to complex/demanding situations.

Work collaboratively in the Multidisciplinary team to plan treatment and care of individual patients.

Work in collaboration with other health care professionals within the wider health community to drive the excellence in clinical care and service improvement within the speciality.

To act as a specialist resource for Stroke patients assessing individual physical and psychological needs and observing the principles of holistic care when planning treatments that are sensitive to the ethnic, cultural and spiritual needs of the patient.

Act as a focal point for support and advice, offering specialist advice whilst supporting patients and their carers to navigate the patient pathway.

Proactively contribute to the achievement of standards/ compliance of targets within their speciality.

Visible presence in clinical area with 85% of job plan being direct clinical care

Provide specialist advice and support to patients and carers from diagnosis and throughout the patient pathway. Liaise with the multidisciplinary team to provide a high standard of care

Responsible for own caseload

Qualifiactions and Training Essential

First level Registered Nurse - current NMC registration Degree or study related to Stroke at level 7 Relevant academic module for Stroke Up to date knowledge of professional issues IT literate

Demonstrate professional development post-registration Evidence of mandatory training updates APAC's Non-medical prescribing Teaching

Knowledge & Experience Essential

Relevant post registration experience at Band 5 or above Highly specialist knowledge in Stroke nursing, underpinned by theory and experience Experience of multi professional working Experience of teaching Experience of leading an audit project cycle

Desirable

Research and development activity Clinical expertise in the specific role Experience running and managing nurse led clinics independently

Personal Attributes & Skills Essential

Excellent communication skills including written and presentation Ability to deal with highly distressing or emotional circumstances Compassionate, demonstrating patient centered approach to care
