Job summary
We are delighted to be recruiting for our dedicated transfer/discharge lounge for our patients at the Grange University Hospital.
This superb initiative reinforces our commitment to deliver outstanding healthcare. The facility is an important phase in the patient pathway to facilitate early release of inpatient beds for new admissions and provides a calm and comfortable environment for patients to complete final treatments and discharge arrangements.
Main duties of the job
The facility provides a nurse led service accepting patients on the day of transfer/discharge ensuring a smooth, timely and safe transition from hospital to home or other specified destination. This role is essential in improving patient flow through the emergency and planned care pathways.
The nursing team will provide direct patient care, whilst co-ordinating the discharge or transfer of patients. This role requires excellent communication and organisational skills.
The facility has the capacity to accommodate 10 patients on beds and 15 patients on chairs and will be open 7 days a week from 8am - 8pm. As a Health Board we support flexible working arrangements and will consider both part-time and full time applications for this post.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
