We are seeking a skilled, enthusiastic and motivated Occupational Therapist with an interest in holistic practice to join the Integrated Reablement Team, including the Urgent Community Response Service across Nottingham City.

You will be part of a multi-disciplinary team including Advanced Practitioners, Physiotherapists, Occupational Therapists, Nurses, Registered Medicine Technicians, Assistant Practitioners, Rehabilitation Support Workers and administrative staff.

The team promotes independence through rehabilitation and crisis management, supporting individuals at home, preventing unnecessary hospital admissions and enabling timely discharges.

We are a dynamic organisation delivering well-recognised rehabilitation services across health, social care, care homes and the independent sector, providing education, advice and support.

An understanding of Health and Social care provision and ability to work closely with partner organisations and other stakeholders is essential.

You will need excellent assessment and intervention skills in order to plan, develop and evaluate goal focussed rehabilitation and Reablement programmes, as well as complex discharge planning experience.

Main duties of the job

The post holder will work as an autonomous practitioner, undertaking comprehensive and holistic assessments to determine the most appropriate service for clients referred.

They will contribute to the multi-disciplinary team in planning and delivering goal-focused rehabilitation and reablement programmes for adults with a wide range of complex physical and mental health needs, either within community beds or in patients own homes.

The role includes maintaining and supporting the ongoing development of high standards of clinical practice with an evidence-based focus, as well as working across the Urgent Community Response and Integrated Reablement Team to respond to health and social care referrals.

The aim is to prevent hospital or care home admissions, facilitate timely discharges from community beds and acute settings, and promote patient independence.

The post holder will be required to participate in rehabilitation treatment programmes alongside the MDT, provide training to other staff members, and support senior colleagues in facilitating student placements.

They must be able to meet the physical demands of the role, including manual handling, and will be expected to travel across the city for client visits, meetings, and training.

Job Purpose

The post holder will work as part of the Urgent Community Response (UCR) and Integrated Reablement Service, delivering high-quality, person-centred occupational therapy assessment and intervention to adults within their own homes or community settings. The role will contribute to the prevention of avoidable hospital admissions, support timely and safe discharge from acute and community settings and promote independence through goal-focused rehabilitation and reablement.The Band 5 Occupational Therapist will carry out holistic assessments, develop and implement individualised therapy and reablement plans, and evaluate outcomes to maximise functional ability and recovery following illness or crisis. Working within a multidisciplinary team, the post holder will support the delivery of responsive, same-day or short-term interventions, enabling patients to remain in or return to their usual place of residence wherever possible.The role requires effective communication with patients, carers and professionals, providing advice, education and support to promote safe, coordinated care. The post holder will work collaboratively across health and social care pathways, contributing to integrated working, safe discharge planning and continuity of care. They will also support service development, quality improvement and the delivery of high standards of clinical practice in line with organisational and professional guidance

Dimensions

Clinical Skills

To be professionally and legally responsible and accountable for all aspects of your own work including the management of clients in your care.To ensure a high standard of clinical care for the clients under your management, and support other team members to do likewise, implementing appropriate teaching, supervision and delegation skills where appropriate.To assess for, provide, demonstrate and fit adaptive equipment using problem solving, and analytical skills.To accept clinical responsibility for a designated caseload of clients, and to organise this effectively with regard to clinical priorities and use of time and ability to be flexible and respond at short notice to the needs of the service.Working within a legal framework, gain informed consent to treatment and have the ability to identify through assessment, clients who lack the capacity to consent.To demonstrate a sound understanding of Clinical Governance and Risk Management and apply to a work situation.To conduct and report on clinical risk assessments as and when appropriate.

Professional

To undertake the comprehensive assessment of clients, including those with a complex presentation using investigative and analytical skills through standardised and non-standardised assessments. Formulate individualised treatment plans with rehabilitation goals using clinical reasoning in consultation with senior therapists as required. This should be based on sound theoretical knowledge, utilising a wide range of treatment skills, and options to formulate a specialised programme of care.To advocate on behalf of clients and carers, where appropriate, overcoming barriers to communication such as cultural or language difficulties, physical or mental health difficulties, using skills of empathy to facilitate or empower the client.To be involved in the development of good practice for physiotherapists within Nottingham CityCare Partnership CIC and Social care under the guidance of senior staff.To complete patient/client records in line with organisation, regulatory body and legal requirements.To represent the service externally and actively participate in the services marketing strategy when required, conducting presentations to a wider audience, including voluntary and public sector agencies.To demonstrate a commitment to evidence-based practice and the use of outcome measures and make recommendations for change. To have a good understanding of the research process, and be willing to participate in local research and audit when appropriate.To be responsible for own Continuing Professional Development, keeping abreast of new recommendations, research and developments, incorporating them into your work as necessary. Attend relevant conferences and external training courses. BSc/Diploma in Occupational Therapy HCPC Registration

Experience Essential

Relevant undergraduate experience working with adults including Health Care of the Elderly and Older peoples mental health / Integrated Medicine, Orthopaedics, Neurology and Musculoskeletal Understanding of multi-disciplinary and/or multi-agency working in a community setting Competence in individual caseload and time management Potential ability to work under pressure, manage caseload. Special Requirements Essential

All appointments are subject to satisfactory medical clearance and the ability to cope with the demands of the role Ability to undertake frequent moderate physical effort for several short periods during the working day.
