Job summary

LNWUH Diabetes & Endocrine Nursing Service is seeking an experienced senior ward nurse who is eager to specialise in diabetes and looking for an excellent opportunity to develop advanced skills within our specialist team across LNWH NHS Trust.

The Band 7 role based at Northwick Park Hospital requires an experienced, dynamic nurse with extensive knowledge and understanding. You will need to be flexible and forward thinking, able to work independently and as part of a busy friendly and supportive team delivering specialist nursing care to a diverse caseload. There will be a strong focus providing high standard inpatient & outpatient care and using innovative virtual ward pathways to reduce hospital admission and length of stay.The post holder will require excellent communication and clinical skills and have previous experience delivering high standard care for people living with diabetes. They will need a commitment to education to ensure the service remains current in developments in the management of people with diabetes.In return we will offer you a comprehensive induction and development programme with robust mentorship and clinical supervision opportunities.

Main duties of the job

The post holder will work as part of the diabetes & endocrine multi-disciplinary team which provides Tier 4 diabetes services to adults and young people between 16 - 19 yrs.

The post holder will be responsible for assessing, developing and implementing specialist endocrine nursing procedures

To help lead the education of patients in an understanding of diabetes and the treatment/management of diabetes and its complications. To develop further partnership working ensuring a smooth interface between the acute hospital and the wider MDT.

The post holder will be a key contributor to the on-going development of diabetes services in line with the NICE guidance, GIRFT and Trust initiatives.

The post holder will work in collaboration with medical and other multi-disciplinary staff to provide a comprehensive service focused care for patients with diabetes and participate in teaching programmes for patients and carers and the direct staff working in the acute setting.

To be committed to the philosophy of patient centred care, aiming to provide seamless, patient focused, and equitable care of all patients referred to the service.

To maintain and develop expert clinical practice in all aspects of diabetes care, having an active role in education and audit in line with national diabetes audit requirements

NMC Registration Evidence of post registration education in diabetes Extensive nurse experience at Band 6 or above in an acute setting Able to work autonomously and within multi-disciplinary team Basic knowledge of diabetes technologies including insulin pump therapy and CGM Ability to deal with complex patients
