Job summary

The role of the Discharge Liaison Practitioner is to support Ward Sisters in providing a high-quality patient focused experience by providing a high level of flexible and reliable administration and support. To assist the ward nurses maintaining discharge records and in ensuring discharge processes are safe and acted upon in a timely manner. The role requires excellent interpersonal, communication, and organisational skills. This role will support the Ward Sisters in maintaining key relationships, including, senior staff, doctors, nurses and allied health professionals, patients, visitors, social services and Primary Care.

Main duties of the job

Liaise with the multidisciplinary team within the ward, discharge lounge the Site Practitioners, AHPs and externally with Social Services, Primary Care, and GPs to ensure the effective discharge planning.

Ensure the white board is up to date and proactively manage delays that may impact on the patient's journey.

Contribute to the effective running of the ward by concentrating on tasks, which directly support and assist the ward sister and the nursing team with regards to safe and timely discharge of the patients. This will include completing discharge planning documentation, communicating with external agencies such as social services, patient's families and carers, arranging appropriate transport and liaising with occupational therapy and physiotherapy.

Answer queries from patients / carers / relatives and provide reassurance regarding their discharge; referring to qualified staff as required.

Ensure patient confidentiality, high standards of conduct and behaviour is maintained at all times.

Education and qualifications Essential

Diploma/NVQ Level 3, GSCE English Language

1-2 years experience with acute hospital setting or care background
