Working Hours: Between 7:00am and 6:30pm
We are seeking a motivated and organised Lead Care Navigator to join our team on a part-time basis. This is a key non-clinical role that will work alongside the current Lead Care Navigator to provide leadership and support to the Care Navigation Team, ensuring effective cover and service delivery across Monday to Friday.
The successful candidate will play a vital role in helping patients navigate the healthcare system by coordinating care, managing patient information, and ensuring individuals receive the appropriate support, resources, and guidance.

Main duties of the job

Support and oversee the daily operation of the Care Navigation Team. Provide guidance, leadership, and support to team members. Coordinate patient care and signpost patients to appropriate services. Maintain accurate and confidential patient records. Communicate effectively with patients, healthcare professionals, and external agencies. Ensure a high standard of patient service and administrative support.About You:The ideal candidate will demonstrate:

Outstanding telephone and communication skills. Previous administrative experience, ideally within a healthcare or customer service environment. Key Responsibilities:
Support and oversee the daily operation of the Care Navigation Team.Provide guidance, leadership and support to team members.Assist patients in understanding their healthcare options and available services. Coordinate appointments and follow-ups with healthcare providers. Maintain accurate records of patient interactions and care plans using data entry skills. Utilise Microsoft Office and SystmOne for documentation and communication purposes. Manage administration tasks such as repeat prescriptions, new registrations and general enquiries. This list is not exhaustive. Provide administrative support to the healthcare team as needed. Demonstrate professionalism in all communications, both over the phone and in written correspondence. Strict Confidentiality is required within this role. We require flexibility on days & hours, which are to be agreed between Monday to Friday 7.00am to 6.30pm.
Person Specification
Qualifications Essential
Previous office experience is essential to ensure smooth operations within the role. Previous experience in a GP Practice would be preferable. Previous experience of managing Staff. Strong computer skills, including proficiency in Microsoft Office. Excellent organisational skills to manage multiple tasks efficiently.. Demonstrated ability to maintain confidentiality and handle sensitive information with discretion. This role requires a proactive individual who is committed to enhancing the patient experience through effective navigation of healthcare services.
