Job summary
We have two positions available, both part time - 25.5 hr and 17 hrs.
The Care Navigator plays a crucial role in providing support and guidance to individuals navigating the healthcare system. This position involves coordinating care, managing patient information, and ensuring that patients receive the necessary resources and assistance. The ideal candidate will possess strong organisational skills, motivation, excellent phone etiquette, and a background in administrative tasks.
Main duties of the job
Duties
The role is for 25.5 hrs and 17 hrs and we require flexibility on days & hours, which are to be agreed but will be between the hours of 7.00am to 6.30pm - Monday to Friday. Assist patients in understanding their healthcare options and available services. Coordinate appointments and follow-ups with healthcare providers. Maintain accurate records of patient interactions and care plans using data entry skills. Utilise Microsoft Office and SystmOne for documentation and communication purposes. Manage administration tasks such as repeat prescriptions, new registrations and general enquiries. This list is not exhaustive. Provide administrative support to the healthcare team as needed. Demonstrate professionalism in all communications, both over the phone and in written correspondence. Strict Confidentiality is required within this role.

Previous office experience is essential to ensure smooth operations within the role. Strong computer skills, including proficiency in Microsoft Office. Excellent organisational skills to manage multiple tasks efficiently. Proficient typing skills for accurate data entry and documentation. Prior Reception experience is preferred to support administrative functions effectively but not essential. Demonstrated ability to maintain confidentiality and handle sensitive information with discretion.
