Job summary

Tobe responsible for the coordination and discharge of Frail Elderly patients on MAU, ED, Wards101, 306, 204 andfSDEC

To work alongside the FEAT team in providing Comprehensive Geriatric Assessment.

To ensure the provision of excellent clinical care and effective management of patient discharges througha holistic approachto assessment of their individual needs, including co-ordination & liaison with both Primary and Secondary multidisciplinary health and social care teams.

Main duties of the job

To ensure patient discharge care needs are assessed, planned implemented and evaluated using research evidence asappropriate.

Be responsible forensuring frail older people are accuratelyidentifiedon admission to the Trust and flagged appropriately.

Tofacilitatea seamless and safe discharge of patients backin totheir own environment or new one that meets their present needs.

Maintain effective communication with the MDT ensuring active participation with nursing and medical teams.

Maintaining personal contact with patients, relatives and carersandbeing sensitive to their needs for courtesy, dignity and privacy andensuring a friendly environment at all times.

Ensuring that accurateandtimelyrecords of discharge planning assessments,interventions and contacts aremaintained.

Maximising the utilisation of the electronic WhiteBoard, using it as a basis for everything in terms of safe discharge and transfer of patients.

Prioritiseown workload and ensure effective time management strategies are embedded within the culture of the FEAT team.

To ensure all nursing or therapy procedures are carried out in accordance withDerby Hospitals NHS Trust policies takingappropriate actionin emergency situations.
