We're looking for an Advanced Clinical Practitioner (Paramedic or Nurse background) to join our supportive and collaborative team at Wem and Prees Medical Practice, working 27 hours per week.

This is a fantastic opportunity to work at an advanced level within a dynamic primary care setting, where you'll bring your clinical expertise, judgement and decision-making skills to the forefront. You'll play a key role in delivering high-quality, patient-centred care-assessing, diagnosing, treating and managing patients with a wide range of needs.

From acute presentations on the day to supporting patients with long-term conditions, you'll be an integral part of a team that works collaboratively to get patients to the right place, first time.

Main duties of the job

This is a role where you'll genuinely feel like you're using the full range of your skills and being trusted to do so.

As an autonomous practitioner, you'll work independently when needed, but always as part of a supportive and approachable multidisciplinary team. You'll bring your clinical expertise, judgement and critical thinking to each patient interaction, delivering care that is safe, effective and evidence-based.

You'll collaborate closely with colleagues across the practice, contributing to how we do things and helping shape improvements in patient care along the way. We value ideas, and everyone has a voice here.

There's also scope to get involved in wider initiatives, clinical supervision and reflective practice because we believe development shouldn't stop once you're in post.

Based across two sites in the market town of Wem and the village of Prees, we care for around 12,000 patients and have built something that feels both busy and genuinely joined-up.

We're part of North Shropshire PCN, giving you access to a fantastic extended team including Physiotherapy, Cancer Care Coordination, Pharmacy, Social Prescribing and Mental Health support. Job description Job responsibilities

Autonomous practitioner who is able to work independently and in conjunction with other members of the MDT.

The ACP will understand and demonstrate evidence-based practice and have a broad knowledge of clinical governance, enabling then to work within their scope of practice.

The ACP will be an experienced caring practitioner with highly developed clinical skills and be able to demonstrate critical thinking whilst working within professional boundaries

The ACP will work collaboratively with the MDT to support delivery of policies and procedures

Participate in local initiatives to enhance service delivery and improve patient care

Take an active role in clinical supervision and reflective practice

Be educated to masters level, this education will include the following core areas: comprehensive physical assessment of all body systems across the lifespan, history taking and clinical decision-making skills health and disease, including physical,sociological, psychological, and cultural aspects of history taking, public health, epidemiology, health education and promotion

Contribute positively to innovative change, be flexible, responsive to new ways of working

Person Specification

Qualifications Essential

Practice within the four pillars; Advanced Clinical Practice Leadership Facilitation of Education and Learning Evidence research and development Registered with a professional body Be educated to masters level or working towards it Be an Independent Prescriber Paediatric post graduate training or equivalent

Desirable

Teaching post graduate training or equivalent Teaching post graduate qualification Covid, Flu, Pneumonia vaccine, trained, PV/PR examination training, dermatoscope training

Experience Essential

5 years post graduate experience Knowledge of working in a primary care environment Autonomous working Reflective practitioner, experienced in clinical supervision management Evidence of CPD, to maintain up to date and evolving competencies Understanding of how primarycare income is generated including QOF Competent in secondary care/2ww/community-based referrals

Desirable

To be aware of audits and their place in primary care
