About this role

Job summary

The Patient Navigation Care Co-Ordinator is a non-clinical role designed to support patients navigating patient pathways across health and social care. Initially, the post will undertake a project based on helping patients navigate the diagnostic pathways for ADHD and Autism but will evolve based on the needs of the patient population and PCN priorities related to tackling health inequalities. You will be the primary point of contact for adults, children, young people and their families considering a diagnosis of Autism or ADHD, guiding them through pre-assessment screening, managing the administrative aspects of referrals and providing compassionate signposting while they wait.

Main duties of the job

You will take referrals from GP surgeries, social prescribers and other teams within the PCN, and work collaboratively with schools and other support agencies where necessary.

You will be knowledgeable in the NHS Right-to-choose pathway and work closely with the social prescribing team and young person's triage and outreach development worker to signpost to relevant support services for patients and their families.

You will need to be self-motivated, able to work autonomously and have excellent written and verbal communication skills.

You will be a strategic thinker with the ability to work collaboratively across several multidisciplinary networks to fulfil the Chesterfield and Dronfield PCN vision.

Experience in General Practice is desired, and you may have already completed the NHS 2-day National requirement personalised accredited care coordination course.

You should be able to work flexibly and adapt to the rapidly changing environment that is primary care.

The successful candidate will be expected to work and travel between Chesterfield & Dronfield PCN practices where necessary.

Job description Job responsibilities

Main duties and responsibilities:

1. Take referrals for patients (aged 5+) who are wanting to navigate the ADHD and Autism assessment pathway.

2. Support patients and their families to choose an appropriate provider for assessment.

3. Support patients and their families to prepare relevant pre-assessment questionnaires and paperwork.

4. Support patients and their families to navigate the onboarding process of the chosen provider.

5. Provide information and guidance to patients and their families about the "Right to choose pathway" and answer any questions.

6. Work closely with the social prescribing lead to recognise when it is appropriate to refer onwards for further support.

7. Work closely with the social prescribing team to signpost patients and their families to community and 3rd sector support while awaiting diagnosis.

8. Manage a dynamic short-term caseload of patients to support through the ADHD and Autism assessment journey.

9. Work as part of a multi-disciplinary team, attending MDTs and CSTs where appropriate.

10. Ensuring that work is delivered in a timely and effective manner, the role will require managing and prioritising your own workload, in accordance with the needs of the patients.

11. Build relationships with staff within the local PCN, attending relevant meetings, becoming part of the wider network team.

12. Work sensitively with people, their families and carers to capture key information, enabling tracking referrals into the service and outwards to other services.

13. Take an active role in quality improvements and innovations, e.g. audits, significant events analysis and development of protocols

14. To undertake general administrative duties to support the role.

Person Specification

SKILLS, KNOWLEDGE & ABILITIES Essential

Understanding of and commitment to equality and diversity in the workplace and in the delivery of services. Can organise and prioritise workload. Understanding of local health and social care services. Ability to listen, empathise with people and provide person-centred support in a non-judgemental way. Ability to work effectively with colleagues, patients and external organisations. Ability to assess risk, anticipate difficulties and successfully address them. Effective communication, both verbally and in writing.

Desirable

Knowledge of the local voluntary sector and local services. Familiar with Safeguarding. Knowledge of the NHS right to choose diagnostic pathway.

OTHER Essential

Willing to work flexibly within the team to cover annual leave or sickness, and to contribute to the extended hours' service if required. Ability to travel within the requirements of the role, with access to own transport. DBS clearance

Qualifications Essential

QUALIFICATIONS, TRAINING & KNOWLEDGE NVQ Level 3, Advanced level or equivalent qualifications or working towards 8 GCSEs Grade A to C or equivalent qualification

Desirable

Training and working knowledge of safeguarding adults and children Previous experience of Care Coordination. Successfully passed the 2-day personalised accreditation care coordination course.

Experience Essential

EXPERIENCE Experience of working within health, community, social care or voluntary sector. Experience of computer software packages including excellent use of Microsoft Word and Excel. Experience of working with adults and children on a 1:1 basis.

Desirable

Experience of using SystmOne or any other case management systems. Experience of working within primary care. Experience of working in a multi-disciplinary team. Experience of working with voluntary organisations. Experience of producing individual care plans. Experience of managing a caseload. Experience of supporting people in a paid or unpaid capacity
