Job summary
To provide a high standard of care and practice at all times, taking direction from Care Team Leaders.
To work as part of an Integrated Care Team and wider multi-disciplinary team respecting the skills, expertise and contributions of others in the provision of holistic care and support to babies, children, young people and their families using Derian House.
To treat each baby, child and young person as an individual respecting their dignity and providing care that is sensitive to their needs.
Main duties of the job
Main Responsibilities
Work under supervision ensuring the provision of high-quality care and practice
To accurately monitor and document the care for the children and young people and ensure that the care provided adheres to individual care plans.
To be responsible for monitoring, and recording any changes in the condition of the babies, children or young people being cared for, including health, emotional and social care needs.
Administering medicines and pharmaceutical products to children & young people following Derian House Children’s Hospice policy, standard operating procedures and NMC standards as directed and supervised by registered staff.
Act in accordance with relevant legal, ethical and professional legislation and frameworks.
To assist, as requested and appropriate with the clinical audits to enhance the delivery of care. Contribute positively to maintaining and implementing the required clinical standards.
To maintain appropriate clinical records ensuring that they are stored securely and confidentially.
Quality Assurance
Participate in the formation of risk assessments for babies, children and young people in your care, reporting all incidents, accidents and near misses, actively participating in any corrective action required to minimise future risk.
Adhere to the prevention and control of infections code of practice (Health & Social care Act 2008) and Derian House Children’s Hospices policies and procedures to prevent infection.
Communication
Effectively communicating the dissemination of information accurately and timely to support effective team working.
Work collaboratively within multi-disciplinary/multi-agency teams both internally and externally to Derian House through the establishment of effective communication systems to support the delivery of high quality care.
Produce clear and consistent written documentation in accordance with professional standards, e.g., NMC and regulation bodies.
Professional
Ensure that personal actions and conduct apply with hospice policies and procedures.
Derian House holds the expectation that each employee will actively participate within mandatory training and successfully complete all relevant role related competency frameworks, within individually agreed timescales.
To maintain confidentiality in all aspects of the role, notably information regarding the children and their families, staff and the organisation itself. Disclosure to any unauthorized person is considered a disciplinary offence.
Health and Safety
Ensure all equipment is utilised and stored safely and ready for use to support the needs of children & young people whilst at Derian House. This will include the timely reporting of any faulty equipment.
To report and record all accidents and incidents in line with hospice procedure safeguarding the welfare and safety of all staff during working hours with regard to the Health and Safety at Work Act.
Ensure health and safety requirements in relation to the provision of care to babies, children, young people and families are managed correctly through the provision of appropriate risk assessments.
Comply with the requirements of Health & Safety legislation, adopting a preventive approach towards the health & safety of babies, children, young people and their families, colleagues, volunteers and visitors.

Main Responsibilities

Hours: 37.5 hours per week (full-time). Weekend, nights and evening work will be required. Part-time hours will also be considered.

Selection and Assessment process: Depending on the volume of applications there may be an initial preliminary 20-minute discussion, held virtually (MS Teams), to help sift candidates we wish to progress. The formal process for those invited, will be an onsite face to face assessment and interview

Person Specification
Professional/ Qualifications & Special Training & Education Essential
NVQ level 3 in Child Care or equivalent.
Minimum 3 Years Experience in a Clinical/ Health or Social Setting that supports complex/ additional Needs in children and young people.
Evidence of the desire to commit to continued professional development.
Required to undertake training to meet all Derian Care clinical competencies within a defined timescale.
Ability to be supportive of colleagues, children and families
Recognition of own limitations.
Desirable
Community Nursing/ Support Experience.
Knowledge/Proven Ability Essential
Proven ability of working with children/young people in a health setting.
Knowledge of child care issues.
Desirable
Proven ability in the Palliative care setting or caring for children/ young people with special complex care needs.
Multidisciplinary team working.
Proven ability of assisting with clinical audit and evidence based audit.
Knowledge/proven ability of working in a hospice or working for a charity.
Skills & Abilities Essential
Evidence of a well-practiced communication skills; both verbal and written.
Evidence of ability to work through competencies in all core clinical skills.
Ability to record information in a concise, accurate and legible manner.
Relate well with children, families and other healthcare professionals.
