About this role

Job summary

To work as part of a team, alongside a wider multi-disciplinary team respecting the skills, expertise and contributions of others in the provision of holistic care and support to Babies, Children, Young People and their families using Derian House.

To be responsible for the assessment of the BCYP’s care needs, including the development, implementation and evaluation of programmes of care.

To assist in the management and organisation of work as required by the Team Leader, ensuring effective communication is maintained at all times and to provide support for junior staff within the hospice.

Main duties of the job

Clinical

Ensure that care for babies, children and young people is person centered, assessed, planned, delivered and evaluated respecting babies, children and young people’s daily routines and promoting independence as far as possible.

To be responsible for providing safe and effective care in line with the hospice’s operating procedures and guidelines and act as a resource for all members of the multi-disciplinary team providing specialist professional, care and support.

To be responsible for improving and maintaining the physical and mental health of the babies, children and young people. Ensure barriers to achieving this are reduced wherever possible and appropriate.

To participate in the primary assessment for newly referred children.

Ensure the respect, dignity and rights of babies, children, young people and their families are upheld at all times.

To be responsible and accountable for the safe administration of medicines within the hospice ensuring medicine management policies and procedures are adhered to as per NMC guidelines.

Clinical/Quality Assurance

Ensure the respect, dignity and rights of babies, children, young adults and their families are upheld at all times.

To be responsible and accountable for the safe administration of medicines within the hospice on a shift basis ensuring medicine management policies and procedures are adhered to.

To adhere to Infection Prevention Control (IPC) Policy and Procedures to ensure safe care is delivered.

Contribute to the day to day management of quality assurance issues reporting and recording incidents, responding to complaints and feedback in line with Derian House policy and standard operating procedures.

Support the organisation by conducting risk assessments for babies, children and young people within the hospice, reporting and recording incidents, accidents and near misses, actively participating in any corrective action required to minimise future risk, as requested.

Contribute to the safeguarding of babies, children & young adults, and identifying any areas of concern and reporting them in a timely way to the Social Worker or a senior member of the team.

Communication

Regularly attend team and other meetings as appropriate, contributing positively to discussions and debates and participating in shared decision making.

Ensure effective communication including the dissemination of information accurately and timely.

Work collaboratively within multi-disciplinary, multi-agency teams both internally and externally through the establishment of effective communication systems.

Act at all times as an ambassador for the organisation and maintain positive and effective communication both internally and externally in representing the service.

To identify the need for pre bereavement care and support working in partnership with the Family Support Team.

Professional

Act at all times in accordance with relevant legal, ethical and professional legislation and frameworks (i.e. Children Act, NMC Code of Conduct).

To take responsibility for maintaining own professional development and undertake further education if required.

Ensure that personal actions and conduct comply with hospice policies and procedures.

Maintain confidentiality in all aspects of the role, notably information regarding the children and their families, staff and the organisation itself.

To work without supervision ensuring personal accountability in accordance with the Nursing and Midwifery Council (NMC) code: Standards of conduct, performance and ethics for nurses and midwives.

Foster a culture of respect and consideration between all team members, respecting and nurturing the strengths, skills and knowledge of others as well as sharing own knowledge and skills.

To attend regular meetings with the multi-disciplinary team to discuss individual babies, children and young people, respecting and inviting the sharing of skills and expertise.

Maintain relevant, accurate and contemporaneous records and reports, respecting and maintaining confidentiality at all times in accordance with NMC Guidelines and the Data Protection Act (1998).

Health and Safety:

Report and record all accidents and incidents, safeguarding the welfare and safety during working hours.

Responsible for the safe storage, transcribing and administration of medicines and pharmaceutical products to babies, children & young people.

Responsible for the provision, monitoring, storage and safe use of equipment.

Take reasonable care to avoid injury to yourself or to others and to co-operate.

Adhere to the prevention and control of infections code of practice (Health & Social care Act 2008).

Our team is small, skilled and deeply committed, and you’ll play a vital part in delivering exceptional care both in our hospice in Chorley and in the wider community.

As our service grows and our 3–5 year plan takes shape we’re creating new opportunities for nurses who want to develop and thrive. We are investing in rotational roles that work across our inhospice and community services, giving you the chance to build breadth of experience in a genuinely varied role. We invest in our people, offering access to an inhouse clinical education team, a dedicated clinical skills lab, and ongoing professional development tailored to your aspirations. Person Specification

Professional/ Qualifications & Special Training & Education Essential

RSCN, RN (Child), RGN, RNLD & RMN with a proven track record of paediatric competency

Minimum of 3 years experience in a clinical setting in acute

Evidence of complying with NMC revalidation requirements.

Evidence of continued professional development.

Required to undertake training to meet all Derian House clinical competencies within a defined timescale.

Desirable

Community Childrens Nursing Experience

Palliative care experience or qualification.

Advanced or Enhanced Communication Skills Training

Leadership Essential

Demonstrable passion and enthusiasm for Derian House and the support it offers.
