We are excited to offer an excellent opportunity for an experienced Practice Nurse to become part of the friendly and supportive clinical team at Rothwell & Desborough Healthcare Group.
The post holder will provide high-quality, evidence-based nursing care to meet the needs of the practice population as an integral member of the multidisciplinary healthcare team. The role will support the delivery of holistic, patient-centred care with a strong emphasis on health promotion, disease prevention, chronic disease management, screening, and patient education.
Working across Rothwell Medical Centre and Desborough Surgery, the successful candidate will join an established and collaborative team committed to delivering excellent standards of care.
The post holder will work autonomously within their scope of practice and will work collaboratively with clinical, management, and administrative colleagues to support the achievement of practice objectives and the delivery of safe, effective, and responsive patient care. As a training practice, you will also contribute to providing a positive learning environment for students and trainees.
Main duties of the job
Provide high-quality nursing care in accordance with NMC standards, professional codes, and practice policies.
Lead the management of patients with asthma and COPD, delivering structured reviews, monitoring, education, and personalised care plans.
Undertake chronic disease management reviews in line with local and national guidelines.
Perform cervical cytology in line with NHS screening programme requirements.
Provide contraceptive advice and management, including routine reviews and prescribing where appropriate.
Accurately record all consultations and interventions in line with NMC and practice protocols.
Recognise and respond to safeguarding concerns, following local and national procedures.
Support vaccination programmes, including childhood immunisations and annual influenza.
Deliver health promotion, NHS Health Checks, travel health, and other screening initiatives.
Educate and support patients with lifestyle changes and self-management of long-term conditions.
Recognise and manage medical emergencies, including anaphylaxis, and perform CPR in line with UK guidance.
Maintain infection control standards, monitor clinical stock and cold chain processes.
Ensure confidentiality and GDPR compliance at all times.
Work collaboratively with the multidisciplinary team to deliver coordinated, effective patient care.
About us
We are a 2 site practice located in the towns of Rothwell and
Desborough (1 mile apart). Our list size is around 22000 and increasing.
We currently have 7 GP partners, 8 salaried GPs, as well as our nursing team including nurse practitioners.
The practice is supported by a management team and very experienced admin, secretarial and reception teams.
We are growing our team. We are looking for a practice nurse work alongside our growing team.

Support GPs and Nurse Practitioners in delivering effective, holistic care for patients with chronic diseases in line with Quality and Outcomes Framework (QOF) requirements. Contribute to the development and review of practice policies, protocols, and clinical guidelines. Provide clinical advice, guidance, and day-to-day support to members of the nursing team. Promote and maintain high standards of care through shared learning and professional support. Support the development of newly appointed nurses through mentoring, knowledge sharing, and clinical guidance. Delegate tasks appropriately, ensuring competence and safe practice. Work collaboratively with the practice team to support the development of clinical knowledge and skills. Contribute to the achievement of QOF and enhanced service targets through effective clinical practice. Undertake opportunistic screening where clinically indicated. Deliver core treatment room services, including venepuncture, ECGs, and wound care management. Perform ear examinations and syringing as appropriate. Provide family planning assessments, advice, and appropriate referrals. Undertake cervical screening and promote breast awareness. Administer adult and childhood immunisations under Patient Group Directions (PGDs), ensuring safe handling and storage of vaccines and medications. Maintain up-to-date knowledge through accredited training in chronic disease management. Contribute to the maintenance and accuracy of disease registers, including asthma and COPD. Provide lifestyle advice and support patients in behaviour change, including referral where appropriate. Deliver travel health services, including vaccinations and patient advice. Identify and respond appropriately to safeguarding concerns, including child protection, vulnerable adults, and substance misuse, following local procedures. Support the use of safeguarding tools, including the child safe trigger tool. Adhere to infection prevention and control procedures, including specimen handling, waste disposal, and management of exposure incidents. Maintain competence in emergency care, including anaphylaxis management and resuscitation, with regular updates.
