About this role

Job summary

You will be responsible for delivering immediate and continuing nursing care to patients within their own homes. You will prioritise, assess, plan, evaluate and deliver all relevant aspects of nursing care to an agreed and expected high standard.

You will work as a key member of the team and contribute to the processes that promote patient safety and quality of care in line with clinical governance frameworks.

Gain competence in clinical skills to enable delivery of quality patient care within the service.

Take an active role in contributing to developing nursing practice.

Maintain effective communication internally and externally to the organisation.

You will be expected to participate fully in their personal development and review process in order to achieve the knowledge and skills identified for this post.

We are recruiting two part-time staff members, each working 26 hours per week.

Main duties of the job

To take responsibility for assessing, planning, implementing and evaluating evidence-based care for patients.

To care for palliative care patients and there families in their own homes.

To provide ongoing nursing care and support to patients and carers, promoting the role of the nurse and clinical excellence.

To identify patients whose conditions become unstable and liaise with doctors and acute services appropriately.

To be responsible for documentation in line with current practice and policies.

To ensure that patients can make an informed choice about their current and future care needs. Ensure that care is patient centred, and confidentiality is maintained in line with Trust policy.

To prioritise own workload and work autonomously when necessary.

To provide support, guidance, and supervision to junior staff.

To assess the level of ongoing nursing needs for people who may need long term care.

To liaise with the patients GP to provide sufficient information for ongoing primary care to be provided safely and efficiently.

Person Specification

Qualifications & Professional Registration Essential

Degree / Diploma in Nursing studies or equivalent Up to date professional portfolio demonstrating evidence of reflective practice Registered Nurse with current NMC registration (Part 1 Adult / General Level 1 or 2)

Desirable

Leg ulcer trained or willingness to undertake. Willingness to complete the palliative care passport V150 prescribing

Knowledge, Training & Experience Essential

Ability to plan and organise own work Awareness of community working and care pathways Computer literacy skills e.g. Microsoft Word and literature searching skills Communication and listening skills Ability to undertake clinical observations Ability to assessment patients needs and plan their care Knowledge of long term conditions and end of life care Ability to maintain own professional development using competency skills framework and on-going portfolio

Desirable

Management of patients with long term conditions e.g Diabetes Management of patients towards end of life Management of patients with catheters Relevant Community experience Sound knowledge of health policy relating to Community Care
