Job summary To be responsible for organising and prioritising own and others workload in the day-today allocation of work along with forward planning and delegation relating to service delivery. To deputise when required as the managers for the team of clinical staff which may include registered nurses and health care assistant. To organise team off duty and annual leave and ensure others are not disadvantaged. To ensure that Health roster and e-expenses are completed and submitted on time. To have organisational knowledge relating to trust protocols and procedures and ensure these guide staff. To be able to deal with minor grievances and be guided by trust policy where appropriate. To be responsible for providing accurate documentation for audit purposes.
Main duties of the job To act as an autonomous, registered practitioner who is legally and professionally accountable for own unsupervised actions guided by the professional code of conduct and Trust guidelines and protocols. The post holder will have Current Effective Status on the Nursing and Midwifery Council (NMC) Registered Nurse. To be responsible, and accountable, for service delivery to clients/patients. To be able to autonomously assess and develop specialist plans of care to meet the complex needs of patients with a variety of conditions. This includes chronic, acute and palliative care within own competencies, recognising own limitations and seeking advice when necessary. This will include continuously evaluating and acting on outcomes. To be able to initiate referrals to other health professional specialist services and agencies. To provide patients and relatives with information and education thus ensuring they have meaningful choices that promote dignity, independence and quality of life. To ensure practice is supported by research, evidence-based practice, literature and peer review. Responsible of patient allocation and staff rotas.

An opportunity has arisen at Integrated Care Team (ICT North) Chingford, Waltham Forest for a District Nurse/ Caseload Manager band 6 to join the team.
The Band 6 District Nurse/Caseload Manager is responsible for the clinical leadership, coordination, and management of a defined community nursing caseload within Waltham Forest ICT North. The post holder will deliver high-quality, evidence-based nursing care to patients in their own homes and community settings, promoting independence, preventing hospital admissions, and supporting timely discharge from acute services. Reporting to the band 7 team lead.
The role involves autonomous assessment, planning, implementation, and evaluation of care for patients with complex physical, psychological, and social needs. The post holder will lead and supervise junior staff, support multidisciplinary working, and ensure safe, effective, and patient-centred care delivery aligned with NELFT values and community service objectives.
Key Responsibilities: Manage a community nursing caseload independently, prioritising and coordinating care for patients with complex health needs in their homes and community settings.
Deliver high-quality clinical care, including assessment, care planning, treatment, and evaluation for a wide range of nursing needs such as wound care, medication management, long-term conditions, palliative care, and catheter care.
Provide clinical leadership and decision-making support for complex cases, ensuring safe, effective, and patient-centred care.
Supervise and support junior staff, including Band 5 nurses, nursing associates, healthcare assistants, and students through delegation, mentorship, and professional development.
Work collaboratively with multidisciplinary teams (GPs, therapists, social care, hospitals, and specialist services) to coordinate holistic care and prevent unnecessary hospital admissions.
Maintain accurate clinical documentation and ensure compliance with Trust policies, safeguarding procedures, infection control, and professional standards.
Contribute to service improvement and quality assurance, including audits, incident reporting, and implementing best practice across the team.
Promote patient independence and wellbeing, supporting individuals and carers to manage health conditions and remain safely at home.

Starting with NELFT NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

High Cost Area Supplement – Outer London (B&D, Redbridge, Havering and Waltham Forest, CEME) This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,714 to a maximum of £5,941 p.a. pro rata for part time).
